This document summarizes a service learning activity where the student attended an online lecture on foundations for clinical proficiency in adult medicine and medical recording. The student reflected on how the activity helped improve their history taking and physical examination skills, allowing them to be more efficient, comprehensive, and standardized in their approach. The student also discussed how they aim to apply what they learned to adhere to professional standards and respect patient privacy in their future work.

Reflection on the Attainment of the Program
Program Outcomes Outcomes During the Activity 1. Demonstrate clinical competence Going back when I was still in the school setting, our professor in adult history and physical examination made us submit a video on how we do our history and PE. Back then, my output takes about 1.5 hours or more, depending on the patient I found in the community. But I learned to do it comprehensively thanks to my former pediatric residents. They taught us to step by step, from broad to narrow. In the end, from formerly 1.5 hours, I learned to write my history and PE for only 30mins per patient. Now I just have to work on how to do certain maneuvers which I can do on the adults. 2. Communicate effectively I was told by my former adult history and PE professor that “I always show compassion and tenderness” in my return demonstration videos which is why my written output has a story to tell because I can draw information from my patients naturally. During this clerkship, I hope I can maintain that kind of skill. 3. Lead and manage health care teams I always believe that if I can do my history and PE efficiently, I can lead my patient to a better approach to treating her disease. For the past two months of my clerkship, I began to see what the doctors tell me that, 80% of the patient diagnosis comes from history and PE. 4. Engage in research activities At the moment, I am still learning ways of enhancing my history and PE. I would always go back to my readings on how to do the maneuvers. Although I am still not that good, I am continuing my quest in learning them and how I make them easier so I could remember them and perform well. 5. Collaborate within interprofessional This week's SLI with the department of internal teams medicine helps a lot. I get to review which part of history taking am I not proficient in or which aspect of the physical examination must I enhance so I could also elicit the results that I need. It was nice that I have something to compare my learnings in pediatrics also. 6. Utilize systems-based approach to For a patient to be given the appropriate therapy, health care our history and PE must be organized as much as possible. Too much information sometimes confuses me which part of the history will I put what she was saying. Every time that I make my history and PE, I do my best to be systematized. When I forget some aspects of my physical examination, I would do my best to go back to it without trying to interrupt my flow or being confused. 7. Engage in continuing personal and I have shortcomings in history and PE. Once, professional development twice, sometimes more than I could count. But as I commit mistakes, I make sure that I learn from them, and the next time I encounter them again, I will know how I should react or adapt to the situation. Adaptation happens a lot when doing history and PE. Because every patient is unique, you should adjust according to the patient so your work will proceed smoothly. 8. Adhere to ethical, professional, and With the lecture last Saturday, I now have a legal standards standard when doing my adult history and physical examination. In the future when I get to be assigned to the OPD or emergency sector of the hospital, I could use what I learned and be able to make a history that is acceptable to my superiors and to the standards of the institution. 9. Demonstrate nationalism, By making an appropriate and comprehensive internationalism and dedication to history and physical examination, I get to show my service dedication to my work as a future physician. After all, I should be able to get to know my patients and deliver them the best course of treatment as much as possible. 10. Practice the principles of social The history and physical examination forms accountability contain all the information that the patient has. Sometimes the cases are too rare or interesting that we forget our patient's privacy. I would keep in mind that not all exciting encounters are to be shared without care.
