AFFIDAVIT OF UNDERTAKING

I, Faye Ruth Ben-ek of legal age, Filipino, residing at Tadiangan Tuba Benguet,
after having been sworn to in accordance with law, hereby depose and state:

1. That I am currently working as one of the Telemarketers of Teledudes which

offers telemarketing service for insurance agents in the United States of
America;

2. That my job description includes but not limited to providing outbound

telemarketing services for Teledudes’s Clients in the United States of America;

3. That I will be dedicated to provide service to only one client of Teledudes at a

given time and should render 8 hrs. of service per day on a 5 day work week.

4. That I am a Freelancer who is paid by way of commission basis and is

contingent on the number of appoint ent or live transfer provided to the client;

5. That systems called Ricochet, Phone burner and Slack were created in order for
the Telemarketers to have access on information with respect to lead’s email,
phone number and address as long as they are still connected;

6. That I undertake to use information accessed therefrom solely in the pursuit

of my responsibility covered by my job description and not for an illegal
purpose and prejudicial to the interest of my co-Telemarketers, Managers and
Company;

7. That all information gathered from the client’s system should only be used for
official agency business and information should not be disseminated to anyone
not connected to the agency I am servicing.

8. That I undertake to refrain and desist from accessing the same following my
severance as Termination either voluntarily or termination for a just cause;

9. That I undertake not to use my personal e-mail to communicate with clients;

10.That I undertake not to offer telemarketing service to current and cancelled

clients;

11.That I undertake not to communicate with the clients regarding personal

matters;
 12.That this undertaking shall remain in force even after my severance with the
Company;

13.That failure on my part to perform and comply with the above undertakings
will not only result in my severance from the Company, but also is meted the
penalty of One Hundred Thousand Pesos (Php. 100,000);

14.That I undertake to ensure performanceof the above undertakings failing

which I shall voluntarily sever my connection with the Company and pay the
corresponding penalty without need of demand.
