DECLARATION OF CONFLICT OF INTEREST PURSUANT TO CLAUSE

7.9 OF THE EMPLOYEE CODE OF CONDUCT AND CLAUSE 1.7.2 AND

1.7.9 OF THE HUMAN RESOURCES AND POLICIES AND
PROCEDURES OPERATIONAL MANUAL.

I of house number having been appointed a

member of staff of the Ghana Deposit Protection Corporation declare that I have no interest that
would constitute a conflict of interest nor engage in any work, paid or unpaid, that creates an
actual or potential conflict of interest .

That I shall not engage either directly or indirectly in another employment which may conflict
with the interest and business of the Corporation or derogate my ability to render unrivaled
services to the Corporation.

That I certify further that I shall fully disclosed to the CEO, to the best of my knowledge, all
circumstances under which I, my immediate family, assigns and or agents have a conflict of
interest with the Corporation and or have an interest in a project which the Corporation engages
in any way whatsoever and or contracts with.

I shall also disclose to my supervisor any likelihood of conflict of interest in my dealings with
Suppliers ,Consultants engaged by the Corporation or member institutions of the Corporation.

I further agree to fully disclose to the Corporation any such circumstances that might arise during
my employment upon becoming aware of such circumstances.
