Dear Sir/Madam,
2. Please be informed that your request for entry into Malaysia has been APPROVED by the Director General,
Immigration Department of Malaysia.
3. This permission is granted subjected to the following conditions prior to entering Malaysia:
i. Download and activate the MySejahtera mobile application as well as register all the requisite
information.
ii. Fill in a Letter of Undertaking and Indemnity (LOU) and email it to the Embassy of Malaysia at least
three (3) days before departure date: and
iii. For countries that require visa to enter Malaysia, you are required to apply for a visa at the nearest
Malaysian Embassy/ High Commission/Consulate General in advance before entering Malaysia.
iv. Complete your online payment for the Quarantine cost and the Covid-19 Screening Test prior to
arrival through MySafeTravel (www.safetravel.myeg.com.my) / MyQr application by MyEG
(www.myeg.gov.my) and provide the payment receipt at the international entry point upon arrival.
4. You are also required to provide the following information to the Immigration Officer upon entry and must comply with
the following conditions upon arrival:
III. Undergo a seven (7) or fourteen (14) - days quarantine at a Quarantine Station stipulated by the National
Disaster Management Agency (NADMA) and Ministry of Health (MOH);
IV. All costs (COVID-19 PCR test and expenses during quarantine shall be borne by the applicant. Failure to bear
the quarantine costs or any monies due to the Government of Malaysia, the applicant’s active pass will be
cancelled, blacklisted in Immigration system and issued with Notice of ‘Not To Land’ (NTL) for deportation;
and
V. Adhere to procedures of entering Malaysia and existing immigration regulations. Applicant must immediately
refer to nearest Immigration Office to apply for appropriate pass.
5. Following the declaration of COVID-19 as a path by the World Health Organization (WHO), you are under the Control of
Prevention and Control of Infectious Diseases Act 1988 and the Police Act 1967. Any person who disobeys any lawful
order, directions or restrictions imposed by any authorised officer or has breached any rules or regulations made under
the Control of Prevention and Control of Infectious Diseases Act 1988 commits an offence under of the said act. Please
be aware that any false reports / statements / representations are an offence under the Section 56(1)(f) Immigration Act
1959/63, and is punishable with a fine amounting to no more than RM10,000.00, or imprisonment of no more than 5
years, or both
7. This letter is valid for six (6) months from the date of issuance
