DepEd School Forms


On January 30, 2014, the Department of Education released DepEd Order
(D.O.) 4, s. 2014, entitled “Adoption of the Modified School Forms (SFs) for Public
Elementary and Secondary Schools Effective End of School Year 2013-2014”, which is
further amended through D.O. 58, s. 2017.
The Deped Order contains the discussions of the different school forms to be prepared
by the teachers in order to organize the data of the learners and the teachers.

The matrix below identifies the persons in charge for each school form.

1. SCHOOL FORM 1 (School Register) - this contains all the information of the
students enrolled in one classroom. This can be considered the official
enrollment form of those who are attending a specific class. Each class and
section is allotted one form. This is ideally prepared by the class adviser through
the Learner Information System (LIS) accessible online, and is prepared at the
beginning of the school year. Below is an example of the SF1, however there
have been some adjustments due to the pandemic.

2. SCHOOL FORM 2 (Learner Daily Attendance Report) - this is the list of the
learners’ daily attendance. This includes the recording of attendance, absence,
or tardiness (template with name of learners). The class adviser is the one to
accomplish this form. Each day, there is a box allotted with a slash in the middle.
The upper part represents the morning session, and the lower part is the
afternoon session. If a student is absent for the morning, the upper part is
shaded black, and if absent in the afternoon, the lower part is shaded. If a
student is absent for the whole day, the whole box is shaded. This is
accomplished every month.

At the end of the form, there is a summary


for the data of the month, as well as the
guidelines on how to compute the data.

3. SCHOOL FORM 3 (Books Issued and Returned) - this is mainly accomplished


by the Public Schools, which include the list of books (by title) issued to and
returned by learners. The class adviser is the one to accomplish this form at the
beginning and the end of the school year.

4. SCHOOL FORM 4 (Summary Enrollment and Movement of Learners) - this is


accomplished by the school head after collecting the SF 2 of ALL grade levels
per month. This is the Enrollment count, transferred in/out and dropout by grade
level, also considered as the summary of SF 2.

The details for the month are summarized according to grade level, as well as
the cumulative counting the previous month. This also includes the following
details:
- Summary of attendance
- Summary of Dropped Out
- Summary of Transferred Out
- Summary of Transferred in
This is a very intricate and important form to accomplish because this will be the
basis for the enrollment count in the LIS and the date recorded on the students’
dropping out or transfer out.

5. SCHOOL FORM 5 (Report on Promotion & Level of Proficiency) - this is one


of the most important forms to be accomplished by the class adviser at the end of
the school year. This contains the complete list of promoted/retained students by
class. The adviser lists the general average of all students (in whole number),
and whether the student is promoted, conditionally promoted or retained. Aside
from that, the list of subjects where the student/s are conditionally promoted or
retained are included in the table. A summary for the categories of the general
average is seen at the bottom of the table. There is a different form for the Senior
High School.

6. SCHOOL FORM 6 (Summary Report on Promotion) - this is prepared by the


School Head or LIS Coordinator at the End of the School Year. The form includes
the number of promoted/retained by grade level, which is based on the SF5. So
in order to accomplish this form, one needs ALL of the SF5 of each grade level.
The list from grade level also includes the summary of the learning progress and
achievement.

7. SCHOOL FORM 7 (Inventory of School Personnel) - this includes the list of


school personnel with basic profile and teaching load/assignment. The school
head prepares this manually by gathering the data of all school personnel,
including their educational background and schedule of classes (for teachers).

8. SCHOOL FORM 8 (Learner Basic Health Profile) - this consists of the data on
the learner assessment of Body Mass Index. This is accomplished by the class
advisers with assistance from the School Nurse or MAPEH Teachers. A summary
is also included at the end of the form.

9. SCHOOL FORM 9 (Learner Progress Report Card) - this is the individual


academic, behavioral and attendance report by quarter (formerly Form 138).
The class adviser is the once accomplishing this form every quarter. The school
is free to develop their own format for this, as long as the information is complete.

10. SCHOOL FORM 10 (Learner’s Permanent Academic Record) - this is the


Individual academic record by quarter and SY (simplified and standardized from
former Form 137). The class adviser is the one to accomplish this form at the end
of the school year. This form summarizes the grades of the students for the
whole year, distributed into four quarters.

GRADING SYSTEM
(D.O. 8, S. 2015)
This DepEd Order focuses on the Policy Guidelines on Classroom Assessment for
the K to 12 Basic Education Program. It includes the different assessment formats,
the cognitive process dimensions, and processes of assessment in the classroom. For
the purpose of this module, we are to focus on the grading system.

Table 3 shows the different components of Summative Assessment. Summative


assessment measures the different ways learners use and apply all relevant knowledge,
understanding, and skills. It is usually conducted after a unit of work and/or at the end of
an entire quarter to determine how well the learners can demonstrate content
knowledge and competencies articulated in the learning standards.
There are 3 components of Summative Assessment:
A. Written Work Component ensures that students are able to express skills and
concepts in written form. Written work, which may include long quizzes, and unit
or long tests, help strengthen test-taking skills among the learners. Through
these, learners are able to practice and prepare for quarterly assessment and
other standardized assessments. Other written work may include essays, written
reports, and other written output.
B. Performance Task Component allows learners to show what they know and are
able to do in diverse ways. They may create or innovate products or
performance-based tasks. Performance-based tasks may include skills
demonstration, oral work, multimedia presentations, and research projects.

C. Quarterly Assessment measures student learning at the end of the quarter.


These may be in the form of objective tests, performance-based assessment, or
a combination thereof.

The K to 12 Basic Education Program uses a standards-based and


competency-based grading system. These are found in the curriculum guides. All
grades will be based on the weighted raw score of the learners’ summative
assessments. The ,minimum grade to pass a specific learning area is 60, which is
transmuted to 75 in the report card. The lowest mark that can appear on the report
card is 60 for Quarterly and Final Grades.

The following are the steps in computing for the Final Grades:
Step 1: Grades from all student work are added up. This results in the total score for
each component, namely Written Work, Performance Tasks and Quarterly Assessment.
Raw scores from each component have to be converted to a Percentage Score. This is
to ensure that values are parallel to each other.
Step 2: The sum for each component is converted to the Percentage Score. To
compute the Percentage Score (PS), divide the raw score by the highest possible score
, then multiply the quotient by 100%. This is shown below:

Step 3: Percentage Scores are then converted to Weighted Scores to show the
importance of each component in promoting learning in the different subjects.
To do this, the Percentage Score is multiplied by the weight of the component found in
Table 4 for Grades 1 to 10, and Table 5 for Senior High School. The product is known as
the Weighted Score (WS).
