January 5, 2020 up to April 10, 2021 and worked as Private Caregiver for
Elizabeth E. Canonoy, 65 years old who has Hypertension and
Osteoarthritis. She rendered eight (8) hours per day (8:00 a.m.-5:00 p.m.),
five (5) to six (6) times a week.

During this period, her services were found to be satisfactory in

carrying out the job duties, her responsibilities were to:

 Administer medications, treatment plus other caregiver services

according to Physician instructions condition of the patient.
 Direct patient in good health habits.
 Give information to family in treatment of patient maintenance of
healthful environment.
 Carried out doctor legal orders.
 Accompany in monthly check-up at the hospital.
 Monitoring of the blood pressure of the patient.

whatever purpose it may serve her.
