PART I
Title of the Activity: School Retrieval of Self- Learning Modules (SLMs) for
Quarter 1, Week 1
OBJECTIVE:

NARRATIVE REPORT
To ensure the safety of both teachers and students, Mapanao Elementary School held its
first retrieval of SLMs for Quarter 1 Week 1 last October 9, 2020 at exactly 8:00 in the morning
following the schedule set per grade level. In order to retrieve the SLMs without endangering
teachers and the students, the manner of retrieval was the same as the distribution last week
where parents/guardians are the ones who will retrieve the SLMs to the teacher adviser.
Safety health protocols were strictly observed to ensure the safety of the parents and the
teachers like wearing of facemask and face shield. Disinfecting foot bath, thermal scanner and
alcohol dispenser were placed at the school entrance. Health declaration form was also
accomplished by the parents in case a person has shown symptoms and for possible contact
tracing. Seats were arranged 1m apart to maintain physical social distancing. Parents were also
required to bring their own pen to prevent the risk of transmission of COVID-19.
The teacher adviser facilitated the recording of retrieved SLMs by having the parents/
guardians sign in the log book/record book to be kept by the school property custodian and the
teacher adviser. Madam Cleofe L. Dela Fuente, Mapanao Elementary School newly appointed
school head monitored the retrieval of self-learning modules to ensure that the flow and safety
health protocols were being followed.
After all the learning materials were retrieved, each of the learning packets were being
cleaned, sanitized and disinfected carefully by the teacher adviser following the protocols in
disinfection like wearing of face mask, face shield, disposable gloves and the use of UV light to
prevent the spread of the virus. Cleaning and disinfecting of school premises, equipment, contact
surfaces and high touch points were also followed.
