If you do not have an international Ukrainian passport, you will have to attend a biometric appointment. An adult will need to go
with you to your biometric appointment if you are under the age of 16.
Tick to confirm that you have read the guidance below and that you know
what evidence you can provide
Proof of identity for your sponsor and proof of identity for any adult who will
be living with you in the same accommodation in the UK. This must be one of:
Alternatively, if your sponsor or adults you will be living with have an eVisa you
can enter their 'share codes' in the box on this page to prove their identities.
A letter of consent for you to apply from both your parents or legal guardians
Your application may be refused if you do not provide your passport. We may also ask you to provide more documents at a later
date.
Evidence that would help us consider your application will depend on the scheme you are applying under:
A copy of the bio data page of the passport or travel document of your UK-based family member
A copy of the passport or bio data page of your Ukrainian family member's passport or travel document (if you are not
Ukrainian)
Evidence that you were living in Ukraine before 1 January 2022
If you are a child, a birth certificate
