TOPIC : CONTRACEPTION COUNCELING & APPLICATION OF INTRAUTERINE
DEVICE

1. GENERAL OBJECTIVE
After finishing this skill practice, the student will be able to perform
counseling and applying basic knowledge and also reasoning for
application of contraceptive device
2. SPECIFIC OBJECTIVE
At the end of skill laboratory, the student expected to be able to :
2.1 Perform GATHER method counseling, informed consent and understand
the reason in preparing tools for IUD insertion.
2.2 Understanding general and specific physical examination, application
of contraceptive device, documentation and data recording.

3. SYLLABUS DESCRIPTION
3.1. Expected Competencies
1. Contraceptive counseling using GATHER method
2. Explain equipment needed
3. Understand how to perform general and specific examination
4. Understand how to apply an intra uterine device

1. Review the anatomy and physiology of female reproductive organ
2. Review the pharmacodynamic and pharmacokinetic of hormonal
drugs

HOMEWORK
1. Please read the “Alat Bantu Pengambilan Keputusan Ber-KB” (APBK) which
include in the skill lab package”
2. Please do an interview with someone in your home that already having a
birth or already marriage.
3. The interview should cover one of the reason for client visit as mention below
(page 3 of the APBK) :
a. New Client which want to choose contraception methode
b. Client who needs protection from sexually transmitted disease (STD)
c. Client with special needs (teenager, 40 years old client, Post partum
client, Post abortion client, Client with HIV/AIDS)
d. Repeating visit of client with specific contraception methode
4. Record the interview in a video and present in skill lab season next week

Principle of contraception counseling using The
GATHER method suggests the following steps:

1 Greet patient in a warm, friendly manner; help her or


him to feel at ease
Eq :
Ask the patient’s identity
 The patient’s name
 The patient’s age
 The patient’s work
 The patient’s address
2 Ask patient about her or his needs and reproductive
goals; ask about risk for STIs
Eq :
Ask the patient why she want to use contraception, eg
:
- Delaying birth of first child
- Avoiding abortion
- Spacing births
- Completed childbearing

3 Tell patient about her or his choices, explaining the


advantages and disadvantages of all options

Explore history of contraceptive use in patient
- What kind of method she has used?
- How long?
- Side effect
4 Help patient to choose
5 Explain the correct use of the method or drug being
prescribed
6 Repeat important instructions to the patient and clarify
time and conditions of return visit; give written
instructions to patient to review later

II Hormone-containing Pill
1 Asking when is the first day of last menstruation
2 Ask whether the client is breastfeeding less than 6 weeks
postpartum
3 Ask whether the client has experienced:
- bleeding / spotting bleeding between menstruation or after
intercourse.
- jaundice in the skin or eyes
- severe headaches or visual disturbances
- severe pain in the calf, thigh or chest, or swollen limbs
(edema)
- a mass or lump in the breast
- high blood pressure (>140/90 mmHg)
4 Ask whether the patient is smoking
5 Ask whether the client is taking anti-seizure drugs (epilepsy)
6 Provide general information about contraception and its types

3 Ask whether the client has experienced menstrual problem, for
example:
- menstruation in large quantities (more from 1-2 pads every 4

hours)
- a long period (more than 8 days)
- severe dysmenorrhoea, need analgesia and / or bed rest
- bleeding / spotting bleeding between menstruation or after
intercourse
4 Ask whether the client has history of pelvic inflammatory
disease, ectopic pregnancy, sexually transmitted infection or
valvular heart disease
5 Provides general information about advantages and
disadvantages/side effects and things which needs to be
considered about IUD
