1. Laboratory examination:

a. Complete Blood Count (CBC) and Blood typing


b. Urinalysis
c. Chest Xray (PA view)

2. Vaccination card/certificate for COVID 19


3. Process for the Issuance of Medical Certificate:

a. To get an appointment with our College Physician, students must send all laboratory
results / COVID – 19 vaccination cards and other requirements required by your college
to the Health Services Unit via email at: gchealthservices@gordoncollege.edu.ph. ONLY
STUDENTS WITH SUBMITTED LABORATORY RESULTS AND COVID VACCINATION
CARDS WILL BE GIVEN AN APPOINTMENT.

b. Students must wait for an email from the Health Services Unit for their appointment.
Forty students will be accommodated in the morning and forty students in the afternoon,
it will be on a first-come first serve basis.

c. On the day of the appointment, the students are required to come between 7:30 am to
9:30 am only in the morning and 1:00 pm to 3:00 pm in the afternoon for the initial
assessment. Only students with complete laboratory results and vaccination cards will be
accommodated.

d. On the Appointment Day, students shall bring all original results of your laboratory tests
and vaccination card, and have your physical/medical exam with our college physician.

e. After the physical exam, the College Physician will issue their Medical Certificate.
