AFFIDAVIT

I, ANGEL DIA C. PASTORAL, of legal age, single, Filipino citizen and a

resident of Brgy. Puelay, VIllasis, Pangasinan, after having been duly sworn to in
accordance with law hereby depose and states that:

1. That I am currently employed as a secondary school teacher at

Amamperez Agro Industrial High School, Villasis, Pangasinan;

2. That I have been employed as a secondary school teacher at

Amamperez Agro Industrial High School since August 2019 up to present;

3. That I was designated as a Coach for Arnis who will participate in the
2019 Palarong Pang Dibisyon at Mangaldan, Pangasinan

4. That all the athletes of Arnis who will participate in the 2019 Palarong
Pang Debisyon are eligible;

5. I further confirm that all the records of the athletes that was submitted
are true and correct;

6. That I am executing this affidavit to attest to the truth of all the

forgoing;
