States of America after having been duly sworn to in accordance with law hereby depose and say that:

1. I am the same person who caused the execution of this document;

2. I have not contracted any previous marriage in the United States of America or in any other
country;

3. I have never been married and my civil status is single;

4. I am capacitated to marry in the United States of America and in the Philippines;

4. I am executing this affidavit to attest the truth of the foregoing facts to support my intention to
apply for a marriage license and enter into a valid marriage in the Philippines;
