Requirements for Notarial Commission

(Makati City)

TN: All requirements must be in three (3) sets: one (1) original copy and two (2)
photocopies. As to the photocopies, it need not be authenticated.

1. Verified Petition.
Must include:
i. Name and address of the Law Office
ii. Lawyer’s residence address
iii. E-mail address
iv. Contact Number/s (Telephone and/or Mobile)

2. Certificate of Membership from the Supreme Court.

3. Certificate of No Pending Case from the Office of the Bar Confidant.

4. Certificate of Payment of the IBP Dues from the IBP National (National
Treasurer is the signatory here).

5. Certificate of No Administrative and Disbarment Case from IBP National

(National Director for Bar Discipline is the signatory here).

6. IBP Dues O.R.

7. Professional Tax Receipt.

8. NBI Clearance.

9. Three (3) specimens of signatures, two (2) sets: Original and Shorthand
signature.

10. Three (3) 2x2 picture with white background.

11. Colored picture of the Law Office with signage.

12. Colored picture of the lawyer in her/his room/cubicle in the office.

13. Sketch of the location of the office. (It was suggested that instead of the
sketch, better to locate the office in the Google map and take a screenshot
of the same. It should be colored.)

TN: Nos. 2-13 will be made as “Annexes” to the Petition. The fees to be paid will
be Php3,050.00. This will already include fees for publication and as per advice
from the notarial office of the Court, they will be the one who will cause the
publication.
