MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

The AGRICULTURAL TRAINING INSTITUTE-INTERNATIONAL TRAINING CENTER ON PIG HUSBANDRY,

with the office address at Marawoy, Lipa City Batangas, Philippines, herein represented by its OIC-
Training Center Superintendent II, RENE C. SANTIAGO,DVM,MSc. and hereinafter known to as ATI-
ITCPH;

And

OSPITAL NG LIPA with office address at Marawoy, Lipa City Batangas, Philippines, herein represented by
its Medical Director, DR. IMELDA V. SIASOCO,MD and hereinafter known to as OSPITAL NG LIPA.

Witnesseth

WHEREAS, ATI-ITCPH is a Training Center specializing in pig production and related discipline under the
Agricultural Training Institute of the Department of Agriculture of the Republic of the Philippines;

WHEREAS, the ATI-ITCPH is the only specialized center of ATI following the training and extension
principle of “learning by doing”.

WHEREAS, the ATI-ITCPH has been the lead agency in the promotion of technology on pig husbandry
since its establishment through various extension activities such as training and farm business advisories
among others.

WHEREAS, the ATI-ITCPH needs medical services as its primary goal is to provide high-quality trainings
and produce highly trainees while providing medical, psychological and collaborative support services
that promote individual health and well-being;

WHEREAS, the ATI-ITCPH has been a TESDA-accredited Training Center for Animal Production-Swine
(NCII)

WHEREAS, the OSPITAL NG LIPA is strongly supporting ATI-ITCPH a TESDA-accredited Training Center
for Animal Production-Swine (NCII), embark on a collaboration with ATI-ITCPH under the following terms
and conditions:

NOW, THEREFORE, for and in consideration of the foregoing, the parties herein agree as follows:

1. RESPONSIBILITIES
1.1 ATI-ITCPH shall:
1.1.1 Orient the students/trainees the standard clinical and legal policy of the OSPITAL NG
LIPA.
1.1.2 Inform the OSPITAL NG LIPA two weeks in advance on any adjustments in schedule
of the NCII trainings.
1.2 The OSPITA NG LIPA shall:
1.2.1 Provide medical services to the NC II students/trainees of ATI-ITCPH, as need arises,
1.2.2 Ensure accessible and proper care for the NC II students/trainees of ATI-ITCPH.
1.2.3 Ensure accuracy of information in the record/logbook.

2. TERMS OF PAYMENT
2.1 Must comply with the applicable fees based on the approved Schedule of Fees and Services
of OSPITAL NG LIPA; and in compliance with the classification of patients if applicable, as
provided in DOH classification of patients in government hospitals or local ordinance.

3. EFFECTIVITY
3.1 This agreement shall take effect from 03 October 2022 will terminate on October 2028.
3.2 This agreement can be terminated upon written mutual agreement by both parties.
3.3 This agreement may be subject to renewal by mutual agreement in writing on or after the
termination of the contract. Any modifications in terms on conditions hereof shall mutually
agreed upon in writing.
IN WITNESS WHEREOF, the parties hereto affix their signature on this 3 rd day of October 2022 at
Marawoy, Lipa City, Batangas.
