JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, NILO A. PAGARAN, Filipino, of legal age, married and a resident of Garcia
Hernandez, Bohol, Philippines, and VIRGILIO D. SALVA, Filipino, of legal age, married and a
resident of Garcia Hernandez, Bohol, Philippines, after being sworn in accordance with law,
hereby depose and say:

That we are the family friends and thus personally know the person of the late
DEMETRIO JAMITO RANARIO who died intestate on February 26, 2015 at Manaba, Garcia-
Hernandez, Bohol;

That the late DEMETRIO JAMITO RANARIO was a member of the United Coconut
Planters Life Assurance Corporation (COCOLIFE);

That the late DEMETRIO JAMITO RANARIO has two children namely; TEOFILO A.
RANARIO and JACINTA A. RANARIO;

That his two children, TEOFILO A. RANARIO and JACINTA RANARIO AMOLATO
(deceased) are his declared beneficiaries in his United Coconut Planters Life Assurance
Corporation (COCOLIFE) Insurance;

That her daughter, JACINTA RANARIO AMOLATO died on November 18, 2013 at
Guindulman, Bohol;

That by virtue of these presents, we hereby attest that the late DEMETRIO JAMITO
RANARIO was survived by his son and legal heir, TEOFILO A. RANARIO;

That we know of such facts because we are neighbors and close family friends of the
late DEMETRIO JAMITO RANARIO;

That we are not related to the said persons either by affinity or consanguinity and that
we have no interest in whatever outcome this presents may bring;

That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
to support the claim for burial and death benefits of TEOFILO A. RANARIO with the United
Coconut Planters Life Assurance Corporation (COCOLIFE) in connection with the death of his
father, DEMETRIO JAMITO RANARIO;

That further, we execute this affidavit for whatever legal purpose this may serve.
