AFFIDAVIT

I, Kabir Mir S/0 Gh Qadir Mir R/o Doodhkul Haril Tehsil Qalamabad District Kupwara
Kashmir do hereby solemnly affirm and state as follows:-

1. That I am permanent resident of Jammu and Kashmir UT.

2. That I intend to avail all the benefits of Kissan credit card (KCC) scheme launched by the
Govt. and abide by all the provisions/guidelines.
3. That I am rearing dairy animals/poultry on scientific lines for production purposes devoid of
drug residues/apparent diseases and my farm produces is fit for human consumption.
4. That I will continue to follow all standard operating procedures (SOPs) as laid down by the
competent authority/authorities mandated for the wellbeing of flock/stock.
5. That I will continue to practice disposal of farm wastes (dead corpse excreta farm waste
aborted foetus placenta act in in any way not inconvenient to general public and environment.
6. That will notify the animal husbandry deptt. Kashmir about any disease outbreak and will
not claim any compensation unless warranted by any law
7. That I will maintain all the necessary farm records and cooperate with the deptt. of animal
Husbandry Kashmir for inspection purposes as and when required.
8. That I will always follow farm biosecurity measures during routines rearing and necessary
directives from the concerned deptt. in this regard.
9. That I will immediately inform Animal Husbandry Department Kashmir in Writing
regarding the sale /purchase/disposal/death/birth etc. of/in my flock/stock.
10. That I will abide by all the laws/acts/statutes/rules as laid down by the govt. from time to
time regarding rearing management including breeding and disease and prevention/
Containment marketing Animal Welfare etc.
11. That I will not claim any benefit from Animal Husbandry deptt. in case of animal death and
I shall be liable to pay the amount to the bank.
