1. PURPOSE

The purpose applies to those activities undertaken COMPANY (MIK / DRC) and Subcontractors
(Execution Contractors’) in the management and development of the project and will align with the
Project Execution Plan for the ROF project.ROO PROJECT

2. SCOPE

The scope of this specification is to cover the basic requirements and guidelines for Subcontractors
(Execution Contractors’) QA and QC requirements. This specification covers all the purchase
orders/sub-contracts placed for.

3. DEFINITIONS

Definitions of terms used in this Quality Plan are in accordance with ISO-9000 : 2005 Quality
Management Systems- Fundamentals and Vocabulary, unless otherwise stated in the following. Any
additional terms and abbreviations, which are used in a specific attachment, shall be defined in the
related document: ROP – Rumaila Oilfield Project

4. RESPONSIBILITIES

The Other Contractor is responsible for compliance with this specification, compliance with ISO
9001:2008 and compliance with the requirements of the PO, codes and job specifications.

5. REFERENCE DOCUMENTS

The basic requirements in relation to quality assurance and quality control are established in the
following references.

STANDARD REFERENCES

1. ISO 9004: 2000 Quality Management Systems Requirements Guidelines for Performance
Improvements.
2. ISO 9001: 2008 Quality Management Systems Requirements Fundamentals and vocabulary.
3. PD ISO/TS: 2001 Petroleum, Petrochemical, and Natural Gas IndustriesSector-specific Quality
Management systems- Requirements for product and service supply organisations.
4. ISO 10005 Quality Management Systems- Guidelines for Quality Plans.

6. RIGHT OF ACCESS

During the performance of the works CONTRACTOR reserves the right of access for the purpose of
effecting quality and /or safety audits as well as performing routine surveillance activity in accordance
with the Inspection and Test Plan mark up (see 7.0 below). This access right will need to encompass any
and all manufacturing facilities and also sub-Other Contractors. These same rights of access are also
required for Company and representatives of Company.

All necessary assistance shall be provided to CONTRACTOR and Company personnel including access
to: measuring equipment, technical/engineering documentation reflecting CONTRACTOR approvals;
specifications including referenced National/International standards and codes; and record documents
such as inspection and test results, traceability documents etc.

During any access periods the Other Contractor shall safeguard the health and safety of CONTRACTOR
and Company personnel ensuring hazards are properly identified and preventive measures taken Safety
induction shall be effected as relevant and personnel accompanied at all times .

7.0 QUALITY ASSURANCE

7.1 General

CONTRACTOR requires that the Other Contractor supply quality products in accordance with the
specified requirements to ensure the objective of the construction facilities are operating in safe and
reliable environment and is achieving high availability.

The Other Contractor shall operate a quality management system in conformance with ISO 9001-2008
which is specific to the purchase order and referencing Other Contractor corporate control procedures.

The direct responsibility for achievement of the quality of goods as specified in the purchase order,
including performance inspection and testing, remains with the Other Contractor irrespective of the
fact that the surveillance activity may be carried out by Company or designated representatives.

The Other Contractor’s quality assurance and quality control activities are to generally conduct in
accordance with company specifications.

7.2 Quality plan

The Other Contractor shall develop a Quality Plan, specific to the purchase order, which shall describe
how the contractor’s corporate QHSE system is to be applied to this project with regards to QA/QC
activities. The Quality Plan shall include the methodology for the control of works, reference procedures
and documentation .

7.3 Quality plan requirements

The Other Contractor shall submit the Quality Plan for approval within (30) days of the effective date.
Any revisions to the Quality Plan shall also be subject for approval. CONTRACTOR shall assist Other
Contractor to prepare quality plan.

7.4 Quality plan content and implementation

The Quality Plan shall describe the sequence of activities in order to ensure that the works are
performed from a QA/QC perspective in accordance with the requirements of the purchase order. For
each sequenced activity the Quality Plan shall make reference to the relevant procedure and/or
instruction that maybe additionally required for the specific purchase order requirements.

The Quality Plan shall identify all technical, procedural and organisational interfaces with Company,
Contractor. The Quality Plan shall demonstrate how the interface management with the Sub-Other
Contractors is to be controlled and how the Other Contractors data and documentation requirements
are to be implemented. This includes the interface between Company, CONTRACTOR, Other
Contractors/ Sub-Other Contractors .The Quality Plan shall include organisation charts for Other
Contractors proposed key management team personnel as applicable to all phases of the works
including manufacturing, inspection and testing. The prime responsibilities of such personnel are to be
described in bullet points showing authority, responsibility, duties and inter-relationships.

The Quality Plan shall include a project audit schedule. The Other Contractor shall execute a programme
of planned systematic audits that effectively monitor the vendor’s performance during all project
activities, including sub-vendors. The protocol for auditing should be in accordance with ISO19011
“Guidelines and/or Environmental Auditing” The Other Contractor shall provide, at least, 5 days
advance notice to Company of any planned audits as to permit Company attendance during audit.
Copies of all completed audit reports together with any associated corrective action reports shall be
issued to the company for information within 3 days of the audit.

The Quality Plan shall include an index of all supporting procedures and plans, including a provisional
timescale for their submission.

7.5 Compliance with the quality plan

During the performance of the works the Other Contractor shall comply with the requirements of the
set procedures as per the Quality Plan.

8.0 QUALITY CONTROL

8.1 Inspection and test plan

The Other Contractor shall develop an Inspection and Test Plan (ITP) which will be submitted to
CONTRACTOR and Company for review and approval within 30 days of the effective date.

The ITP shall clearly identify sequential work activities design (as appropriate), procurement,
manufacture and testing of the goods. Each work activity shall reference the quality characteristic which
shall include the clearly stated inspection standard or code to be verified.

The work activity should also reference the relevant clause number of the controlling procedure and
indicating the acceptance criteria with a signature box to verify that the quality characteristic has been
achieved. Deliverables (quality records) related to each activity shall be identified.

8.2 Identification of surveillance activity

The Other Contractor shall submit the ITP to CONTRACTOR for review and approval. During the review
cycle the required inspection notification surveillance by CONTRACTOR and Company shall be identified
and returned to the vendor for incorporation in to the ITP as: Review point; Witness point; or Hold point
as defined below.

▪ Review indicates that the documentation referred to in the ITP shall be


reviewed and endorsed by Company or designated representatives and
CONTRACTOR. Such review may be retrospectively but shall be completed.
▪ Witness point indicates that CONTRACTOR shall be notified of the timing of the
inspection or test, receiving the appropriate notice. The inspection or test shall
be performed as scheduled. If the Company or CONTRACTOR is not present
then the Other Contractors/ Sub-Other Contractors may proceed to the next
activity.
▪ Hold point identifies that a hold shall be applied to the activity and that the
inspection or test shall not be carried out unless the Company or CONTRACTOR
is in attendance.

This can only be waived by CONTRACTOR in writing.

The CONTRACTOR retains the right to modify these agreed surveillance points taking into account of
actual quality performance.

8.3 Coordination & Correspondence

Formal notification shall be by email but telephone contact shall be established to provide further detail
and finalise arrangements for surveillance. Address details, contacts phone and email address shall be
provided separately.

8.4 Other Contractor Approvals

Other Contractor shall obtain necessary approvals, in a timely manner including:

• Technical approval in advance of any related quality control activity


• Other Contractor sign off as competed for each activity identified as requiring Hold or Witness
point surveillance on the Inspection and Test Plan at the time of activity (except where waived).

8.5 Competency

The Other Contractor is responsible for ensuring competent personnel are employed and verifying that
this is supported by relevant qualifications. Any specific requirements identified in the purchase order
or related specifications shall be addressed.

8.6 Miscellaneous

Progressive inspection, not identified as observe/hold points by CONTRACTOR or Company, may be


carried out at various stages in production or manufacturing, in order to monitor compliance with
defined requirements and to observe that the right fabrication procedures are applied and adhered to.

The completed equipment, or packaged item, may also be inspected prior to painting and preparation
for shipment.

Mill test report and /or code forms, required by the order or pertaining to inspection and test plan
activities, must be available for the inspector at the time of final inspection.

Before commencing with any repairs which may affect. equipment integrity or operation, the Other
Contractor will have to issue to Company or CONTRACTOR a repair procedure and obtain their approval.

Materials and equipment that are subject to inspection and are shipped without Company or
CONTRACTOR inspection will be rejected.

8.7 Waiver

Written notice will be given by Company or CONTRACTOR whenever Company or CONTRACTOR will
decide to waive inspection Hold points, partially or totally, for material otherwise subject to inspection.
Test reports and certificates relevant to test not witnessed must in any case be prepared by Other
Contractor’s quality control organisation for review.

9.0 Pre-inspection meetings

A meeting will be arranged, usually at the Other Contractor’s premises for all complex/critical
equipment to review the requirements, this document and the Other Contractor’s data and
documentation Requirements specification to ensure there is a clear and common understanding of
the requirements and how the interfaces between parties are managed.

10.0 Documentation

10.1 General

Documentation shall be submitted as required The specification identifies the quality records required
to be presented for review by Company/CONTRACTOR at Other Contractor works, those requiring
Company /CONTRACTOR Engineering approval and those to be incorporated into he certification
manual.

10.2 Documentation for Inspection

The Other Contractor’s documentation requirements may differ from PO to PO, therefore the required
Other Contractor’s documents, plans and procedures shall be stated in Section IV of the approved
Purchase Order.

All manufacturing drawings and relevant documentation are to be submitted for approval in a timely
manner to comply with the manufacturing schedule.

11.0 CODE AND OTHER THIRD PARTY INVOLVEMENT

Company or CONTRACTOR may request to arrange any third party activity such as associated with
pressure vessels, lifting equipment, type testing, Marine Warranty Surveyor activity etc. ensuring
appropriate Code certification, release notes etc are obtained.

12.0 NON-CONFORMANCE

Company or CONTRACTOR inspection supervisors, as well as their designated inspectors, shall have the
right to reflect any material or equipment found to be defective, or showing poor quality or
workmanship, or that is not in conformance with order requirements. In such cases Inspectors will issue
a nonconformance Reports to the Other Contractor or their Sub-other Contractor.

Upon receipt of the non-conformance Reports , the Other Contractor shall either take immediate action
to replace the defective part or component, or advise Company or CONTRACTOR in writing on proposed
corrective action, or issue a concession request asking for the acceptance of a deviation.

Corrective actions implying repair activities shall be covered by a procedure that must be sent to
Company or CONTRACTOR for review. No repairs may be started prior to the receipt of Company or
CONTRACTOR release relevant to their repair procedures.

When concession have been approved, or corrections have been completed and the achievement of
full compliance with the PO requirements have been verified, Company or CONTRACTOR will transmit
to Other Contractor the “clearance’ or resolution of the pending non-conformance Reports.

No equipment, material and work will be finally accepted unless all pending non-conformance Reports
are not cleared.

Any non-conformance shall be analysed by the Other Contractor to determine cause, potential extent
of the problem and identify any deficiencies.

13.0 INSPECTOR SAFETY

13.1 General

For the sole purpose of safeguarding the health and safety of Company or CONTRACTOR Personnel
during the performance of their activities at Other Contractor premises (e.g Inspection, visits,
expediting checks etc.) Other Contractors are requested to give to visiting Company or CONTRACTOR
personnel, possibly in writing, the following information:

a. Any possible risk for health and safety incurred when operating at the Other Contractor
premises
b. Prevention and protection measures and individual protection means or equipment that
vendors foresee.
c. Procedure relevant to fire protection and fire fighting, evacuation of personnel in case of
serious and immediate danger, rescue, first aid and procedures for the management of
emergency.

Should the situation be modified in such a way as to change the possibly risk for Company or
CONTRACTOR / Company or CONTRACTOR representatives, it will be Other Contractor’s duty to update
the information formerly given with the new risk and the relevant prevention and protection measures.

The Other Contractor is not relieved, by complying with the above request, from the due diligence in
the surveillance on Company or CONTRACTOR personnel presence.

Company or CONTRACTOR personnel will be obliged to strictly comply with Other Contractor’s safety
rules, except when these rules are in conflict with the Law or company or CONTRACTOR safety
standards.

14.0 ERRORS AND OMISSIONS IN SPECIFIED REQUIREMENTS

The Other Contractor, as part of determining CONTRACTOR requirements, will review the information
provided; should any errors, inconsistencies or omissions be evident clarification shall be sough by
Other Contractor from CONTRACTOR.
