Individual Consent/Declaration Form

As part of your Security Vetting Checks you are asked to supply “personal data” as defined by the
GDPR.

You are providing your “personal data” to Electranet and, in some instances, the appropriate Security
Vetting Authority for Security Vetting Checks. The “personal data” provided may be in the form of
written applications such as Baseline Personnel Security Standards Form, Ministry of Justice
Application Form, Non-Police Personnel Vetting Form and on-line Security Vetting application forms
as required for the level of clearance you have been requested to undertake.

The “personal data” you provide will be processed exclusively for the purposes of Security Vetting
Checks, by Electranet and Security Vetting Authorities, including but not limited to, United Kingdom
Security Vetting, Ministry of Justice, Non-Police Personnel Vetting, Access NI, Disclosure Scotland and
the Disclosure and Barring Service. As part of your Security Vetting Checks you may also need to
undertake a face to face ID verification check with one of our trained ID verifiers and provide “personal
data”.

Electranet and the Security Vetting Authorities will protect the information you have provided for
Security Vetting Checks and will ensure it is not passed to anyone who is not authorised to use it.

By signing this Individual Consent/Declaration Form, you are explicitly consenting for the data you
provide to be processed in the manner described above. If you have any concerns about any of the
questions or what we will do with the information you provide, please contact the person who issued
this form for further information.
