1.1. Description

1.1.1. Description for Data Modification Request Form

- The data modification request form is a form that is filled out when a change of data or
a change of a database needs to occur. This form also serves as a checklist. The form is
used to evaluate the preparedness of a system or an organization for change.

1.1.2. Description of Database Change Management Checklist

- The database change management checklist is a checklist used to evaluate the

prepared-ness of an organisation for change of a database system.

1.2. Advantages

1.2.1. Advantages of the Data Modification Request Form

- The advantages of this checklist is that it allows the DBA to fill in the information they
seem necessary, and to provide more detailed and specific answers.

1.2.2. Advantages of the Database Change Management Checklist

- The advantage of the database change management checklist is that the questions are
very specific, and therefore allows the DBA filling in the form, to be very direct with their
answers, as they are only allowed to check yes, no or not applicable (N/A).

1.3. Disadvantages

1.3.1. Disadvantages of Data Modification Request Form

- Too many fields on the form may discourage the DBA from filling the form with full
transparency.

- Interpreting and filling in the information manually may take a while, and this may
encourage the DBA to focus on completing the form quickly rather than giving accurate
answers.

1.3.2. Disadvantages of Database Change Management Checklist

- The database change management checklist may be too narrow, and the contents of
the checklist may only focus on certain aspects of the change management process.
This discourages the DBA from giving more insightful information.

1.4. Thoroughness

1.4.1. Thoroughness of Data Modification Request Form

- The data modification request form is much more thorough because it asks specific
more specific questions and provides space for more thorough answers.

- This checklist is less thorough in nature as it asks straight-forward, direct questions that
require a simple yes or no answer, and occasionally a N/A may be applicable.

1.5. Details

1.5.1. Details of Data Modification Request Form

- The data modification request form focuses on the way in which data existing in the
database is affected and how the DBA is going to manage the data using a new
database system.

1.5.2. Details of Database Change Management Checklist

- This checklist focuses on the procedure followed before migrating from one database
system to the next. The chronological order in which steps/phases of the change
management request are executed is also important in ensuring the right changes are
made at the right time. The first item on the checklist is about mapping proposed
changes before carrying them out. The checklist also contains aspects in which back-
ups are mentioned a few times, meaning this checklist was created with an objective to
ensure there are always back-ups of recent files.

1.6. Conclusion

In conclusion both the data modification request form and the database change
management checklist are good checklists. Each provide a new advantage over the
other as well as a disadvantage from time to time.
