MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is executed by and between:

The SAN ISIDRO NATIONAL HIGH SCHOOL, a public secondary institution, with the
office address of San Isidro, Bacon District, Sorsogon City, represented herein by its School Head,
MODESTO G. EDMA, herein after referred to as “DONEE”.

The _________________ of ______________________,with the address at
_________________________________, represented herein by its ___________________,
_______________________, herein referred to as “STAKEHOLDER”.

WITNESSETH: That,

WHEREAS with the 2022 Brigada Eskwela and Oplan Balik Eskwela, amending thereby DepEd
Order No. 048, s. 2021 titled Brigada Eskwela Implementing Guidelines, the DONEE acknowledges the
need for partners in the opening of classes and Brigada Eskwela 2022 and in the whole year-round:

WHEREAS the STAKEHOLDERS will voluntarily provide fund assistance for the
_________________ (specify project) as one of the school’s initiated Brigada Eskwela projects anchored
on the School’s Improvement Plans.

WHEREAS the STAKEHOLDERS shall show support and initiate by donating in kind
including but not limited to educational supplies, health and sanitizing equipment or materials for
COVID-19 prevention, seeds/seedlings, and paint materials.

NOW, THEREFORE, for and in consideration of the above premises and mutual covenants,
agreements and stipulations hereinafter, set forth, the parties hereby agree jointly to be partners in the
2022 Brigada Eskwela and Oplan Balik Eskwela;

I. ROLES AND RESPONSIBILITIES OF THE PARTIES

1. The DONEE shall:
1.1 Coordinate with the STAKEHOLDERS of the specification of funds for the
_________________ prior to its purchase so that the need of the school shall be
complied;

1.2 Oversee the management, operation, and development of the project and the efficient
utilization of donated monetary funds in the purchase of equipment and materials;

 1.3 Oversee the management and proper accounting of the support given by the
STAKEHOLDERS and ensure that the same are treated well and accommodated
accordingly.

2. The STAKEHOLDERS shall:

2.1 Coordinate with the DONEE on the specification of the support, facilities,
equipment, materials, and other resources needed by the school;

2.2 Coordinate with the DONEE on the monetary support needed for the implementation
of _____________________;

2.3 Purchase the needed materials and other resources for the school and donate the same
for the 2022 Brigada Eskwela and Oplan Balik Eskwela.

II. EFFECTIVITY OF THE AGREEMENT

This agreement shall take effect upon the beginning of 2022 Brigada Eskwela and Oplan Balik
Eskwela and is automatically extended should the 2022 Brigada Eskwela and Oplan Balik Eskwela be
extended by the governing authorities.

IN WITNESS WHEREOF, the parties hereunto have affixed their respective signatures this
