Select the appropriate defect Code from the drop list

Enter the “Defect” by text – Description of the defect.

Enter the “corrective action” - Description of what is required to rectify the defect
Select the “due date” – As a thumb rule maximum time the due date is given is 3 months from the date of creation of the defect, however in special circumstances
where the due date might be required to be postponed due to any reasons (due to lack of spares/stores/postponed till dry dock etc), it will later be required to
extend the due date with appropriate remarks.
Upload the “defect file” – this is the place to upload the picture of the defect (Before rectification).
Once all this data is filled up click the “Create defect” to create the defect on the portal under the vessel.
