1. What kind of questions are mainly used to self-assess the given supply chain?
Why do you think it happens? Explain.
Response:
The questions in the questionnaire are focused on how the supply chain influences
the company and what actions we are taking to avoid having a great economic
impact that could affect it.
This type of question looks for how the supply chain is being managed by logistics
coordinator to show shortcomings in this process and improved to have a better
performance.

 Why do you think it is important to have different types of questions to assess a


supply chain?
Response:
It is important to have different questions when evaluating the supply chain to
evidence the flaws and the opportunities for improvement that can be implemented
in the process.

 What aspects of the supply chain are being evaluated by means of the given
questionnaire? Explain.

Response

The aspects that are being evaluated are: organization, Technology and
commitment, which are important aspects in the supply chain, so that the
process has an excellent organization and distribution applying the best
technologies to obtain a 100% reliable inventory and that the organization It
evidences the commitment of each employee to obtain lower costs and higher
profits.

 What kind of information can be analyzed to improve the current situation of the
supply chain?
Response:

The information that can be obtained is how the organization and employees can
improve the supply chain by applying new strategies that are consistent with the
mission and vision of the company.

2. After analyzing the previous self-a sesenta questionnaire and checking the
complementary material “Sample supply chain management questionnaire”,
create your own questionnaire, including different types of questions to assess
one of the following aspects (select only one):

 Customer experience.
 Customer service.
 Transportation management.

3. Remember to include a short paragraph to explain why this questionnaire is


important for you as the Distribution Manager and for the company itself. The
questions you ask must cover different aspects of the topic being evaluated. You
must create minimum ten (10) questions and maximum, twenty (20). Do not
forget to include open and closed questions. The potential information you
collect, will be useful to make your company be better.
