I, MERCEDES AGOTO RABAGO, of legal age, Filipino and a resident of Alibagu, City of
Ilagan, Isabela, after having been duly sworn to in accordance with law, hereby depose and say
that:

1. I am the same person executing this affidavit;

2. As per Certificate of Live Birth, authenticated by the National Statistics Office

(NSO) now Philippine Statistics Authority (PSA), my registered middle name is
“Agoto”;

3. However, upon looking on my Official Transcript of Records

issued by Isabela State University, City of Ilagan, Isabela my
middle name was “Aguto” ;

4. I hereby certify and swear that the person with the name “MERCEDES AGOTO
RABAGO” which appeared in the Certificate of Live Birth, authenticated by the
National Statistics Office (NSO) now Philippine Statistics Authority (PSA), and
the one with the name “MERCEDES AGUTO RABAGO” which appeared in my
Official Transcript of Records issued by Isabela State University,
City of Ilagan, Isabela, refers to one and the same person;

5. I am executing this affidavit to attest the truth of the foregoing and for whatever
lawful purposes it may serve;
