KNOW ALL MEN BY THESE PRESENTS:

This Contract made and entered into by and between:

GOLDENSTATE COLLEGE, INC., a Higher Education Institution


(HEI) duly recognized under the Commission on Higher Education
(CHED) and the Technical Skills and Development Authority
(TESDA), with principal office and business address at P. Acharon
Boulevard, General Santos City, Philippines, now and hereinafter
called the PARTY OF THE FIRST PART/SCHOOL, and for the
purpose of these presents herebyrepresented by its Chief Operating
Officer/Vice-President for Academics, DR. JOSIE YAP-TIRADOR,
DBA, per Board Resolution and/or Secretary’s Certificate hereto
attached as Annex “A” and made an integral part hereof,

-AND-

GOLDENSTATE LITTLE COLLEGE, a Filipino, non sectarian


educational institution with address located at________________,
Philippines, now and hereinafter called the PARTY OF THE
SECOND PART,

WHEREAS, The Teaching Internship is a learning opportunity, in which the student works in a
professional environment and receives academic credit. Students will be required to complete
500 hours of exposure which is divided into 200 hours Pre-Internship Program and 300 hours
Field Internship via Virtual Modality or Face to Face Modality.
WHEREAS, Internships can vary greatly, there are some general guidelines that will help
students and cooperating instructor identify appropriate internship opportunities and ensure the
internship is a valuable learning experience.
This agreement will identify an appropriate tasks and responsibilities along with amount of work
to be done.

RESPONSIBILITIES OF THE STUDENT INTERN


1. Complete 200 hours requirement for Pre-Internship Program following all instructions as
provided in the Internship Handbook and Work Instruction.
2. Complete 300 hours requirement for 300 hours Field Internship via Virtual Modality or
Face to Face modality following all instructions as provided in the Internship Handbook
and Work Instruction.
3. All undertaking the student intern wish to go through shall require consultation and
approval with the Internship Coordinator. All undertaking without approval shall be
considered null and void.
4. Properly Present oneself during internship. Male interns shall put on formal and decent
outfit, shaved, virtual background must be clean and presentable. For Female interns
shall put on formal and decent outfit, light make up, virtual background must be clean
and presentable.
5. Submit requirements on time and refrain from absences. Students whose absences are
excessive and without notification made to the Internship Coordinator shall be basis for
failure.

6. Submit daily progress to the Group Chat as provided in your work instruction. Failure to
do so means failure to the entire subject.
7. Practice Good Manners and Right Conduct (GMRC) in dealing with people, students,
teachers. Respect and give due consideration to others whenever necessary.
8. Work under the direction of your cooperating teacher.

RESPONSIBILITIES OF THE COOPERATING TEACHER

1. Prepare the classroom students to receive the Student Intern as a professional co-
worker.
2. Prepare a workspace for the Student Intern.
3. Orient the Student Teacher to the students, the school calendar and daily schedule, the
building facilities and resources, the personnel-administrators, faculty, and staff, school
policies and procedures, the nature of the community, and professional opportunities.
4. Provide a gradual induction to the teaching process by modeling appropriate planning
and teaching.
5. Demonstrate methods and resources for creating daily lesson plans that are sequential
and integrated.
6. Choose a specific time for planning on a daily/weekly basis.
7. Guide the student teacher toward effectiveness by giving him/her tasks.
8. Evaluate student intern monthly for 3 months of working side by side with the
cooperating teacher.
9. Provide opportunity to team teach and conduct final demonstration teaching. Provide for
student interns grade which will constitute 60% of the students grade.

RESPONSIBILITIES OF THE INTERNSHIP COORDINATOR

1. Confer with student interns on a continuing basis. Identify students performance


problems and discuss as early as possible.
2. Participate in a three-way evaluation conference with the cooperating teacher at the mid-
point and final evaluation.
3. Submit final evaluation.
4. Compute 40% of the student interns grade.
