ASSESSMENT OF THE LEARNER/ COMMUNITY:

COMMUNITY: The barangay Guadalupe with a population of 50,000 and age group of 30 with the common diseases identified.

Chronic Kidney Disease is the top disorder identified in the barangay which follows with some similarities with some of these following diseases
Kidney stones, Glomerulonephritis, Polycystic kidney disease, UTI (Urinary tract infection). There is no such big hospital here in our barangay that can
accommodate such long term confinement diseases we only have a local barangay health clinic that can accommodate check-ups and more mainly focused
on moms who are pregnant. But we have many clinics which is private and a barangay that has a responsive ambulance that can accommodate an
emergency situation heading back to the topic our barangay has a dialysis center namely metro cebu kidney dialysis center.

The residents of the community will be able to acquire basic knowledge on signs and symptoms, treatment and management of chronic kidney disease. And
will be able to avoid unhealthy lifestyle while maintaining a healthy lifestyle to avoid such disease

EVALUATION:

The client will be able to maintain a normal blood pressure and especially GFR (glomerular filtration rate) and avoid some unhealthy lifestyle to
avoid Chronic Kidney Disease.

Chronic Kidney Disease.
