Considering the validity and reliability of the tools, mark the boxes if the following
information were considered:

The research instrument of the study will be the survey questionnaire as the main data
gathering tool. Item of the questionnaire will be accumulation of adopted parts of different
studies with corrections or modifications as suited in the characteristics of the locale of the
study and the respondents. In addition, a focus group discussion or informal interview may be
conducted prior to the construction of the questionnaire to have a glimpse of the context in
terms of attitude and the perceived challenges towards Modular Distance Learning modality
in learning General Mathematics.

Part 1 of the survey questionnaire will be all about the demographic profile of the

respondents.
Part II will solicit responses on the attitudes of SHS students in General Mathematics during
Modular Distance Learning Modality. Part III will seek responses on the perceived challenges
encountered by SHS students in General Mathematics in Modular Distance Learning
modality.
Part IV will pose an open ended question on the effectiveness of Modular Distance Learning
modality in learning General Mathematics.

In terms of scoring in the survey questionnaire, a four-point Likert Scale will be used.
Descriptions will vary on each independent variable of the study. The survey questionnaire
will be written in both English and Filipino language to ensure that there is no language
barrier in the study.

Moreover, the scores of the test paper with table of specifications in General mathematics
will be used to determine the achievement of the students in General Mathematics.

All the research instruments to be used in the study will undergo validity through field
experts for face and content validity and will be piloted first for reliability testing to the non-
participant SHS students.

Prior to the conduct of the study, letter will be addresses to the school authorities, the parents, and
the participants requesting permission to conduct the study. After the approval of the request, the
researcher will start the validity and reliability test of the questionnaire before it will be
administered to the final respondents. The final questionnaire will then be used to collect the
necessary data. Data regarding the participant’s academic performance will be based on their
general weighted average in General Mathematics obtained last school year and their scores in
achievement test.

The researcher will ensure to establish rapport with the students during the administration of the
questionnaire and explain thoroughly to them each item. Data gathering will be done during the
vacant time of the researcher and the respondents as agreed upon by both parties. The data
collected will be tabulated and screened first to guarantee a valid analysis.

For the achievement of students, a scheduled time will be set as agreed and approved by the school
head to administer the assessment.

The researcher will ensure confidentiality of the responses and data gathered throughout the
