SUBJECT : Explanation re Late Submission of Significant Incident


(Sketchy/Initial and Spot Reports) through Police Operations and
Management Information System (POMIS)

DATE : July 22, 2022

1. Reference: POMIS Generated Reports/Statistics covering the period of


July 20, 2022. (Violation of Sections 5 and 11, Article II of RA 9165 Magalang MPS)

1. In connection with the above subject and reference, be informed the


undersigned have no intention to disregard any directive and fully aware of the
significance of timely reporting of incidents.

2. That, At about 7:49 PM of the same date, I called PSSg Princetone Mutuc
POMIS Encoder of Magalang MPS to informed him that I was approved the
Sketchy (Violation of Sections 5 and 11, Article II of RA 9165) that the
approved was on the record in the system of approved list on POMIS while
the other one is still pending on approval. I check the pending for approval
and I notice and that it was also the same so, that I informed him that I will
disapproved the other pending one because in the monitor of POMIS it was
also the same. But around 8:08 PM of the same date he (PSSg Princetone
Mutuc) called and informed me that approved sketchy report was nowhere
to be found. I checked also the system that when I notice that the approved
sketchy report was not there. I advise him (PSSg Princetone Mutuc) to
encode again but it was already beyond 1-hour period.

3. That, the undersigned has no malicious intent nor neglect or to defy legal
orders from Higher Headquarters and exert relentless/diligent effort to
check all any incident that transpired within through approving of sketchy
report on the Police Operations Management Information System (POMIS)

4. Attached herewith is the screenshot of call logs from Magalang MPS for
your reference.

5. Praying that this explanation be given merit and consideration.
