Narrative Report
(October 5, 2021)

Today is the first day of modules distribution (Module 1-2) for the First Quarter.
As the officer of the day, I see to it that everything is prepared for the smooth conduct
of the said activity. I checked the triage area, placed the scanner with a built-in alcohol
dispenser, and put the registration or triage form where visitors can list their names and
body temperature on it.
With the assistance of the school COVID-19 task force, signages reiterating the
minimum health protocols and standards are placed where can be visibly seen. Prior to
that, everyone is advised to wear a facemask and face shield and to bring their ballpen.
Advisers have also warned the public through their class group chats that minors and
senior citizens are not allowed to come to school. However, due to some reasons, few
do still come to get modules. They were then reminded about the existing school
protocols, and in consideration of the distance they trek before reaching the school, they
were able to get the modules but will not be tolerated next time.
Due to limited access to electronic gadgets and the internet, parents do not know
their child's class section, and some of them were not yet included in the master list.
Some were advised to get an enrolment form from the teacher concerned to officially
enroll their child. Afterward, modules were issued to them.

Narrative Report
(October 12 & 19, 2021)

Since classes were cancelled due to typhoon Maring, there is no distribution and
retrieval of modules on October 12, 2021. It was rescheduled on October 14, 2021
(Thursday). Minimum health protocols were observed to ensure the safety and well-
being of everyone.

DOCUMENTATION:

October 19, 2021 – Distribution of Modules 5-7 and retrieval of Module 3-4 were
conducted. Same procedures were observed in compliance with the IATF guidelines
and health protocols. Stakeholders were also welcomed and received their donations to
school. To show the school’s gratitude to them, their photos and donations were posted
in its official FB Page.
