I, PSSg Mary Jane P Songgadan - 232559, of legal age, bona fide member of
the Philippine National Police presently assigned at Manila Police District, Moriones
Tondo Police Station (PS-2), after having been duly sworn in accordance with law to
hereby depose and state;

1. That, undersigned assigned as Intel Operatives at Moriones Tondo Police

Station (PS-2) was directed to explain for being absent during conduct of inspection by
PMAJ EDWIN A MALABANAN on failure to attend and testify in court on September 5,
2022.

2. That the undersigned has no intention to defy any lawful orders.

3. That the undersigned was not able to perform her tour of duty due
to verbal instruction given by PMAJ JOSEPH DIZON JIMENEZ, Chief, Intel to finish
immediately and submit the compliances and data that will be presented by our Station
Commander in the scheduled meeting he will be attended.

4. In view of the above, respectfully prays reconsider the above

stated explanation that the said respondent be absolved from any administrative liability.

Tondo Police Station (PS-2), after having been duly sworn in accordance with law to
hereby depose and state;

2. That, undersigned assigned as Intel Operatives at Moriones Tondo Police

Station (PS-2) was directed to explain for being absent during conduct of inspection by
PMAJ EDWIN A MALABANAN on failure to attend and testify in court on September 5,
2022.

3. That the undersigned was not able to perform his tour of duty on
the said date of inspection due to experiencing symptoms of flu.

3. That the undersigned, informed his immediate supervisor on his

condition and was advised by the latter to just stay at home and finish the important
compliances that will be presented to our Station Commander so no one will get
infected in the office.

5. In view of the above, respectfully prays reconsider the above

stated explanation that the said respondent be absolved from any administrative liability.
