NARRATIVE REPORT ON MATERNAL DEATH

Mrs. Maite Limbago Alvarado 31 years old born on May 7, 1991 married to Mr.
Almerante Alvarado Jr, A resident of Purok 7 Dona Carmen , Tagbina, Surigao del
Sur. Blessed with 3 children all females Named Miralyn L. Alvarado born on april
24,2009, Mecaila L. Alvarado born on april 23, 2012 and Melanie L. Alvarado born
on june 16, 2014. Mrs. Alvarado is G4P3A0 during interview with her mother she
has no history of hypertension and Diabetes no history of severe blood loss during
port partum, she is also a current user/ continuing of COC pills. On month of
february Mrs. Bayani a BHW assigned in the purok 7 told her to visit for medical
check up and early prenatal care as she verbalized” dili ko buntis te kay nag pills
man ko naa man cge ako regla nabalaka ko kay cge ko spotting” On March 3,
2022 she visited Dr. Arreza and prescribed pampakapit. On March 23, 2022 the
fetus was delivered via home delivery on the following day March 24, 2022 she
visited Dr. Arreza for follow up post partum visit and Dr. Arreza was on Seminar
on that day she was advised to visit on the next day March 25, 2022. The
Dilatation and curettage was performed at Andres Soriano hospital and advised
to ready 4 bags of blood because she has a low hemoglobin count only 2 bags was
infused due to lack of supply. Mrs. Alvarado was admitted almost 1 week. After
she was discharged almost 1 month she is stayed home she is manifesting edema
on both feet, Skin bruises, On and off fever, cough and pain on the site of
anesthesia was administered. On May 17 she was admitted at Hinatuan district
Hospital as verbalized by the mother 2 bags of compact blood was administered
and there is improvement as the mother says. On the day of 22 nd of May 2022 she
was died with the immediate cause of Anemia Etiology to be determined possible
gynecologic in origin.

May 19, 2022 on the 2 nd day after the dog bite happened Mr. Absin suffering
fever and chill and also difficulty of breathing. He was rushed to Bislig District
Hospital and declared Dead on arrival. I personally visited the residence of the
dog owner mrs. Pajit the dogs are alive.
