UTIVE SUMMARY

This report is a summary of the 1-day invasive fitting survey at XXX


of the XXX Unit-XXX. We conducted an onsite inspection on
the XXrd of XXX 2019.

The scope of work for the inspection was to provide a report on the installation of the
fittings surveyed to ensure the correct installation procedure had been followed.
20% of the overall fittings were inspected.

During the onsite inspection we identified no issues with the installation of the fittings
surveyed.

INSPECTION SUMMARY

After conducting an inspection on 57 random fittings on the XXX


skid I found no defects or issues with the assembly of the swaged connections.

All connections surveyed were made correctly at the time of review and met the typical
installation instructions.

All fittings were re-made prior to the pressure test by the customer.
