AFFIDAVIT OF UNDERTAKING

sworn to, depose and say:
1. That I am the authorized representative of the registered owners of a parcel of land described as
follows:

2. That the property owner intends to have the said parcel of land be reclassified from agricultural to
commercial purposes;

3. That, I hereby warrant to submit to the Sangguniang Bayan of Mexico, Pampanga as soon as the
following required documents have been secured and available from the different offices of the
government, to wit:

a) Conceptual plan;
b) Zoning Certification
c) Municipal Agriculturist Office (MAO) Clearance;
d) Department of Agriculture Certification
e) DSHUD Certification;
f) DAR Certification;
g) NIA Certification; and
h) DENR Certification of alienable and disposable

This undertaking is being executed to attest to the veracity of the foregoing, in compliance with
the requirements for reclassification by the Sanguniang Bayan of the Municipality of Mexico, Province of
Pampanga and for all legal intents and purposes.
