BIDDER shall fill in the table below. It is required to give as much details as possible. In case it is required
to provide any document and/or BIDDER makes a reference to any document in its answer, such
document shall be attached hereto.

BIDDER shall include references to specific page in the bid where relevant documents or info are

etc. As the minimum, you have to provide
copies of valid driving licenses, certificates from
the Iraqi Society of Occupational Health and
Safety for operators of lifting equipment and
permits for slingers.

17 Qualification of the Site Provide resume for your Site/Project Manager


Manager in compliance with the Contract requirements.
We need to know his education, previous
experience, see copies of his diplomas and
certificates, references from employers etc.

18 Acceptance of the Draft Do you unconditionally accept the conditions of


Contract the Draft Contract proposed by the Company,
including conditions for payment?

If yes, complete ATTACHMENT #1 TO


CHAPTER IV of ITB - CONTRACT
ACCEPTANCE FORM.

V If not, specify the provisions of the contract you


cannot accept, explain why, and propose the
changes/modifications to these provisions (see
ATTACHMENT #2 TO CHAPTER IV –
COMMENTS TO DRAFT CONTRACT).

Please note that full acceptance of the draft


Contract provisions will be a serious
advantage.
