Seven Hundred Ninety Six Thousand Pesos (₱ 796,000.00) has been considered as Lowest
Calculated and Responsive Bid.

The price of the above mentioned item awarded to your company will be valid until December
31, 2018. Likewise, you are hereby required to provide within ten (10) calendar days the
Performance Security in the form of Cash, Cashier’s / Manager’s check, Bank Draft/Guarantee in
the amount of Five percent ( 5%) and GSIS Surety Bond or Private Surety Bond callable upon
demand issued by a surety or insurance company duly certified by the Insurance Commission as
authorized to issue such security in the amount equivalent to Thirty Percent ( 30% ) of the total
Contract Price as stipulated in the Instruction to Bidders. Failure to provide the performance
security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid
security.

The East Avenue Medical Center reserves the right to withdraw / terminate this Notice of Award
in view of non-delivery on the time specified, delivery of items of poor quality / items with
expiration date less than eighteen (18) months, substandard and any other violation of the terms
and conditions provided in the instruction to Bidders.
