Please be informed that we are extending the submission of the hard copies of your
Enrollment Requirements until October 28, 2022. Please find below the list of the
requirements that you need to submit based on your student type:

for Freshmen
From Other School:
• Enrollment Information Sheet with 2x2 ID Picture
• Original Report Card/Form 138 signed by the Principal
• Photocopy of PSA Birth Certificate
• Notice of Admission (NOA)
• Certificate of Non-Enrollment (if not a SHS graduate of 2022)
DLSL Senior High School Graduates:
• Enrollment Information Sheet with 2x2 ID Picture
• Certificate of Non-Enrollment (if not a SHS graduate of 2022)
for Transferees
• Enrollment Information Sheet with 2x2 ID Picture
• Photocopy of PSA Birth Certificate
• Original Transcript of Records
• Honorable Dismissal/Transfer Credential
• Notice of Admission (NOA)
for CULINARY
• Enrollment Information Sheet with 2x2 ID Picture
• Original Report Card/Form 138 signed by the Principal
• Original PSA Birth Certificate
• Notice of Admission (NOA)
• Learner’s Profile Form with 1x1 ID Picture
• Trainee’s Profile Form with 1x1 ID Picture
for MBA /JD
• Enrollment Information Sheet with 2x2 ID Picture
• Original PSA Birth Certificate
• Original Transcript of Records
• Honorable Dismissal/Transfer Credential
- DLSL Graduates are not required to submit this form
• Notice of Admission (NOA)
• Certified Copy of Diploma
You may submit the said documents in the Office of the College Registrar from 8:00am
to 5:00pm or you may send it thru courier at the address below.
