CORE FUNCTIONS
Prepare lesson plan/activities for 25% 12 lessons and submit photo
children in Day-in Center documentation every month
Conduct daily activities or lesson 25% 15 activities or lesson and submit
photo documentation with
attendance sheet 5 days
Ensure children’s safety while in 15% No complaint of untoward
Day-in center incidents.

Manage day to day operation of 15% Ensure the safekeeping of


the child care center including documents, equipment, furniture’s,
physical upkeeps. and cleanliness every month
Assist in the conduct of various 5% Monitor and keep track supplies
activities organize (PPEs) Personal Protective
equipment. Filed and Recorded in
the logbook daily. Jan-June 2022
Distribution of PPEs 5% 5 completely filled out distribution
sheets and responded within the
required timeline.

Support Functions
Attend to supervisors’ order from 5% ANA, All request are responded
time to time. within the required timeline.
Performance management 5% 1 IPC & IPCR certificate of
summaries submitted to HRPPMS
per Timeline.
