AFFIDAVIT OF DESISTANCE

That I, NOEL MANUEL R. LOPEZ, of legal age, Filipino Citizen, married and
a resident of Ilagan City, Isabela after having been duly sworn to in accordance with law,
depose and say:

1. That I am the private complainant for a Grave Slander with NPS Docket No.
11-14-INV-176-00149 now pending at National Prosecution Service Ilagan
City, Isabela against SALLY SAMBILAY;

2. That on January 9, 2018, DR. SALLY SAMBILAY went to my office

explained to me her side and apologized;

3. That after talking to her, I came to realize that DR. SALLY SAMBILAY had
no intention of committing any offense against me;

4. That by reason of the above-mentioned circumstances, I am no longer

interested in prosecuting this present case, hence, I am now praying for the
dismissal of this case;

5. That I am executing this affidavit freely and voluntarily to cause the

withdrawal of my foregoing complaint, to support my prayer for the dismissal
of the forgoing case, to attest to the truth and veracity of all the foregoing facts
and for all other legal intents and purposes.

IN WITNESS WHEREOF, we have hereunto set our hands this 12th day of
