Drowning? Nursing Management

Definition
of
drowning
Drowning is defined as a process of
experiencing respiratory impairment
from submersion/immersion in a liquid
medium. To delineate the incident's
outcome, this is further divided into
descriptive terms such as death,
morbidity, and no morbidity. It can be
nonfatal (previously called near drowning)
or fatal.
Death caused by
Fatal drowning

Drowning is a form of
death by suffocation.
Death occurs after
the lungs take in
water. This water
intake then interferes
with breathing. The
lungs become heavy,
and oxygen stops
being delivered to the
heart.
Non- Fatal
drowning
Non-fatal drowning
describes a drowning
incident where the
individual survives. It is
sometimes incorrectly
referred to as 'near-
drowning' – this term has
been replaced by the
World Health
Organization and should
not be used.
Epidemiology

According to the GBD 2000 data, an

estimated 449,000 people drowned
worldwide (7.4 per 100,000
population) and a further 1.3 million
Disability Adjusted Life Years
(DALYs) were lost as a result of
premature death or disability from
drowning. 97% of drownings occurred
in low- and middle-income
countries.
Causes
1. Lack of swimming ability
The most common cause of drowning is not
knowing how to swim. Many adults and children will
attempt to get into the water without proper
swim training.

2. No barriers around the pool

A four-sided fence separating the pool area from
the house and yard reduces a child’s risk of
drowning by 83%.

Causes

3. Lack of supervision
Drowning can happen anywhere there is water.
Never leave a person (particularly a child)
unattended in or near a bathtub, pool, pond, or
even a bucket of water. There should always be a
lifeguard on duty or a competent supervising adult
and never swim alone.
4. Failure to wear life jackets
72% of boating deaths that occurred during 2010
were caused by drowning, with 88% of victims not
wearing life jackets.
Causes

5. Alcohol use
Alcohol use is involved in about 70% of water-
related deaths among adolescents and adults. One
in five boating deaths involves alcohol, according
to the CDC. Alcohol affects your balance,
coordination, judgment, and basic motor skills.

The consequences and symptoms

of drowning vary widely. A drowning
victim may show no symptoms and
have no complaints, or may be
found dead.
•It is the rare person who is found
symptoms thrashing in water. Instead, most
drownings are unwitnessed and the
person is found floating or
submerged in the water.
•For those who are alive, they may
be anxious, confused, and short of
breath. Brain function and lung
function are the main concerns in
drowning victims.
Somesymptoms include:
• Difficulty breathing
•Constant coughing
•Vomiting
•Drowsiness and unusual
symptoms
tiredness
•Shortness of breath
•Fever
•Changes in behavior by
exhibiting greater irritability or a
lack of energy. This is because
less oxygen is reaching the brain.
Nursing management
Assess the client’s level of consciousness.
•Assess the client’s respiratory rate, depth, and rhythm.
to do •Auscultate lung for breath sounds such as crackles and wheezing.
•Monitor for signs of respiratory difficulties such as nasal flaring,
stridor, retractions, and the use of accessory muscles.
•Assess for any signs of worsening pulmonary edema.
•Monitor oxygen saturation and ABGs as ordered.
alert •Monitor chest-x-ray readings.
•Provide suctioning as per client’s need only.
•Maintain the client’s airway and assist with ventilation as needed
while protecting the cervical spine.
•Provide oxygenation as ordered.
