Please Read Carefully

Herewith I declare to follow the minimum requirements on site as specified below
at all times:
 Mandatory PPE (Personal Protective Equipment’s): safety helmet, safety shoes, high visibility
vest, and safety glasses.
 Visiting the construction area, I need always to be attended by an authorized site member.
 It is strictly forbidden smoking on site, except that in the designated smoking shelters, and any
use of alcohol or/and drugs.
 Before leaving the site remember to return the visitor badge and take back your ID card.
 Note: PLEASE ASK IF THERE ARE ANY DOUBTS!

Emergency Procedure & Telephone Numbers

On site there is an Emergency Communication and Emergency Response System controlled by site Security:

1 In any Emergency  your supervisor / co-workers / contractor team

Inform to  Organization Site Security
2 Communication  Through phone
 Organization Security / Emergency Numbers are
positioned at site
3 Evacuation  If evacuation is required / Site Security is informed to do
evacuation, then reach to the nearest Assembly Point
 Stop working, put everything in safe, alert everybody
around you, and make yourself ready for the evacuation.
4 Assembly Points  Once reach the Assembly Point Roll count will be taken
wait for some time.
 Do not move from assembly point until “All Clear”
Communication is given.
5 All clear  Once the Organization Site Security give the “All Clear”
Communication then go back to work

Report all Incident / Unsafe Acts to immediate supervisor / EHS team / Site Security

I declare that:
 I attended the Site Visitor HSE Induction.
 I received all the necessary information about the Emergency & Evacuation Procedure.
 I received the “Health, Safety, & Environment instructions “.
 I received all the necessary information about Site General Security Instructions.
 I know that I have to wear the PPE: helmet, safety glasses, safety shoes, HV vest at all
times on construction site.
 I received all the necessary information regarding my responsibilities
