Excavation/Trench Safety
1.Lack of continuous solid barriers was observed for
deep excavation/trench.
2. No means of adequate access/egress for deep
excavation (minimum one not exceeding 15m
horizontal distance).
3. Excavation crossing bridge was observed blocked
by heap of excavated spoils.

Plant and Equipment Safety - Blinds


Multiple equipment on site were observed with
blinds/curtains blocking the 360-degree view of the
operators. High risk of vehicular incidents - GEC and
CC to ensure close monitoring.

Plant and Equipment Safety – Competency


Front loader equipment with reg. #12389 was
observed lacking an operator’s cabin and functional
waring beacons. On verifying at MOI portal, it was
observed that the equipment registration expired in
the year 2016.

Hole/ Opening Protection


Observed lack of collective protection/ adequate
collective protection for manholes/ openings in the
work zone.
(Note* Even though backfilling activities are in
progress, the manholes shall be covered to prevent
risk materialization.)

Work Equipment Safety – Inspection and


Vibration Control
1. Hand driven plate compactor equipment was
observed without proof of inspection (periodic
maintenance).
2. Concerning hand-arm vibration (HAV) exposure
action value (EAV) and exposure limit value (ELV),

the ELV was not indicated on the equipment (This
should be included with the inspection tag for the
operator to minimize the risk of HAV syndrome).
3. Operative was observed without the job specific
PPE i.e., safety gloves

Man, and Manually controlled Machine Interface


Observed that there was no “signaler/flagman”
assigned to guide multiple equipment maneuver
wherein workers were operational in the vicinity.

Work Equipment Safety – Periodic Inspection
Angle grinder equipment was observed without proof
of periodic inspection (no inspection tag and colour
coding was present).

Personal Protective Equipment


Some operatives were observed without adherence to
mandatory/job specific PPE (hi-vis vests).

Risk of Impalement to the Workforce


Observed steel reinforcements bars used as shutter
supports without protection caps; operatives deemed
to be operational in the proximity of these unprotected
steel reinforcements.
