Cordova Tower Marquinton Residences, Sumulong Highway, Antipolo City, after having been
duly sworn to in accordance with law depose and say:

1. That I am the registered and lawful owner of a certain one (1) unit of Condominium
identified as Unit 707 7th Floor of Cordova Tower Marquinton Residences located at
Brgy. Sto. Niño, Sumulong Highway, Marikina City, covered by Condominium Certificate
of Title No. 009-2016000793, containing an area of 22.10 square meters;

2. That the Owner’s Duplicate Copy of Condominium Certificate of Title with No. 009-
2016000793 was discovered to be missing and that despite diligent efforts having been
exerted the same could not be located;

3. That I have neither endorsed, negotiated, mortgaged, pledged or in a manner

transferred and encumbered the said Owner’s Duplicate Copy of Condominium
Certificate of Title with No. 009-2016000793, nor has cancelled, attached, or levied the
same;

4. That I am executing this affidavit to attest to the truth of the foregoing narration and to
notify the Office of the Registry of Deeds of Marikina City of the said lost and cause the
necessary issuance of the same through the necessary court Petition and for whatever
legal intents and purpose it may serve.

the said Owner’s Duplicate Copy of Transfer Certificate of Title with No. R-580, nor has cancelled, attached,

of the Philippines with office and business address at Shaw Boulevard Kapitolyo, Pasig City, herein represented by
its General Manager MR. SERGIO E. ABCEJO after having been duly sworn to in accordance with law hereby
depose and state: THAT-

1. The above stated corporation is the buyer of the one (1) parcel of land situated in Barrio of Pulo, Sta.
Cruz, Municipality of Sta. Rosa, Province of Laguna, containing an area of 136 square meters, more or
less covered by Transfer Certificate of Title No. T-37367, under the registered name of SPOUSES
MARIANO SARMIENTO and FLORENTINA MONTOYA-SARMIENTO, as per Deed of
Absolute Sale, which is more particularly described as follows:

of land as BUYERS thereof and further warn the public not to enter into any transaction involving the
