DEMOLITION AND REINSTATEMENT CHECKLIST

The issuance of the Notice to Proceed (“NTP”) is dependent upon Tenant’s submission of the requirements included on this
checklist. Without the NTP, no works, including delivery of equipment or materials, can commence.

- Clearly specify names and contact details (mobile, landline, email) of project

- Photocopy of Safety Officer’s ID or certificate.
- I.D.’s will be returned to contractors for lamination.
All workers must submit to Building Management one (1) original and one (1) photo copy
of their individual personal records. Original copies will be returned once validated.

9. Authorization Letter, with three (3) original signature specimens, indicating Tenant is authorizing Fit-Out Contractor to sign Notice to

CARI policy must cover the following:

- Coverage shall be for at least 125% of the total fit-out cost.


- Assured parties include Contractor, Tenant, and Zuellig Builidng
- Public Liability Insurance Policy to cover Tenant’s liability to third parties. The coverage
shall be for at least 125% of the total fit-out cost.
 10. Contractor’s All-Rick Insurance (CARI)
- Accredited insurance company of Landlord is FPG INSURANCE CO.

DEMOLITION REMINDERS

Coordination with Building Management


Coordination with Building Management throughout the construction process must be done by the project manager (from general contractor and/or
Project Management Team). Individual contractors/sub-contractors will not be entertained.

Daily Requirements During Demolition

▪ Notice to Proceed – must be posted on clearly visible location at unit entrance (glass or board-up).
▪ Work Permits– must be posted on clearly visible location at unit entrance (glass or board-up).
▪ Gate Passes
▪ Hot Works Permits – If necessary.
▪ Garbage Bins – Daily hauling of garbage/construction debris is mandatory.

Noisy Works

▪ Noisy works must be scheduled properly. Plan out works and areas where noisy works will commence. Conduct site markings for
ceiling hangers and support, chipping and drilling works, and schedule the actual drilling/chipping in batches to minimize disruption to
neighboring tenants and tenants above and below your floor.
▪ The noisy works schedule must be coordinated with Building Management at least 3 days prior to the schedule as this will be
coordinated by Building Management with the affected tenants.
▪ Noisy works are allowed from 8 p.m. to 5 a.m., Monday to Friday and whole day of weekends.

Ceiling Works

▪ The sprinkler heads will be deactivated/drained in the entire floor during the construction to give way to ceiling works. Therefore, all
works that will or may affect the sprinkler system must be prioritized and completed first (i.e. relocation of sprinkler heads, relocation of
lighting fixtures, installation of partition, ducting works, and all other ceiling works). These must be completed within the first 2-3 weeks
of construction. Non-compliance will result to a fine of PHP10,000.00 for every day of delay.
▪ Once the ceiling works have been completed, water refilling of the sprinkler system must be done and this has a fee of PHP1,500.00,
chargeable to the tenant undergoing fit-out.

Inspections

▪ Random inspections will be done throughout the office reinstatement.


▪ Tenant Contractor must request for an initial inspection before closing the ceiling and raised flooring. During inspection, ceiling and
raised flooring must be free from obstruction. Request for inspection may be emailed to BMO and subject to the availability of Building
Engineers.
▪ Tenant Contractor must request for final inspection and clearance prior to unit hand over. Request for inspection may be emailed to
BMO and subject to the availability of Building Engineers. Ceiling must also be open during final inspection.
▪ Copies of test results duly witnessed by Building Engineering must be submitted to Building Management.
▪ To request for release of construction bond, Tenant’s via their tenant contractor must request for Post Construction Inspection.
Accomplish the Building Request for Final Inspection and Release of Construction Bond Form (ZB_BLDG FORM_009).

Inspections and Testing Procedure


▪ Schedule: Tuesdays and Wednesdays 1:30 p.m. to 6 p.m. (except for tests that must be scheduled after 6 p.m.)
▪ Tenant / contractor must book an appointment at least 2 working days in advance or earlier.
▪ Contractor must use the standard ZB format printed in the contractor’s letterhead.

Protective Coverings

The various violations and infractions listed above shall not be deemed as exhaustive. Bridgebury Realty Corporation (“BRC”) reserves the right to
include additional deductions other than the above, which is deemed fit and is entitled to impose not more than PHP15,000.00 per
incident/occasion as agreed damages as stated in the Tenant Fit-Out and Improvement Manual. The Tenant’s contractor shall take immediate
action to rectify any violations or infractions committed upon instruction by BRC or its authorized representative(s). The amount of agreed
damages shall be compounded for repeated non-compliance.
