2. All interested and qualified applicants, including persons with disability (PWD), members of

a. Fully accomplished Personal Data Sheet (PDS) with recent passport – sized picture (CS Form No.
212, Revised 2017) which can be downloaded at www.csc.gov.ph ;
b. Three (3) recent performance ratings, if applicable;
c. Photocopy of certificate of eligibility/ rating/ license;
d. Photocopy of Transcript of Records;
e. Certificate of trainings/ seminars attended;
f. Service Record;
g. Brief Description of past and present work;
h. Other pertinent documents based on Regional Memorandum No. 178, s. 2020;
i. Organize the documents according to the sequence of criteria in RM No. 178, s. 2020 with proper
tabbing to ensure easier and more precise assessment on the pertinent documents to be submitted.
