SUBJECT : Guidelines on Testing for Tuberculosis (TB) and Coronavirus Disease

(COVID-19) for Laboratories within the National Tuberculosis Control
Program (NTP) Laboratory Network

In 2020, existing laboratories within the National Tuberculosis Control Program (NTP)
laboratory network were tapped as COVID-19 testing laboratories in order to increase the
country’s COVID-19 testing capacity. The rationale for this strategy includes: a) a number of
these laboratories are compliant to the minimum biosafety requirements for a COVID-19
testing laboratory; and b) the GeneXpert instrument, which is needed to run the Xpert Xpress
SARS-CoV-2 assay for COVID-19 laboratory diagnosis, is already installed in these
laboratories nationwide.

As the services of these laboratories shifted to COVID-19 testing, TB testing services were
temporarily suspended. With this, the country’s COVID-19 testing capacity increased, while
the TB testing capacity decreased. Given the current scenario, the Philippines will only be able
to reach around 54% of the 2021 national testing target for TB Culture of 108,900, and 37%
for Line Probe Assay (LPA) (target = 11,880). The country needs to achieve these targets in
order to meet its commitment during the United Nations General Assembly high-level meeting
on TB last September 2018 to find, notify, and treat 2.5 million Filipinos with active TB by
2022.

Thus, in keeping with the DOH’s mandate under the Universal Health Care Act, guidelines and
strategies need to be harmonized in order to ensure efficient laboratory response for both TB
and COVID-19. The guidelines are as follows:

1. The following TB laboratories capable of performing LPA! in strategic regions shall

revert to exclusive TB testing by September 1, 2021:

'LPA, a rapid, molecular-based TB drug susceptibility test (DST), is essential in determining the appropriate treatment regimen
for people with drug-resistant TB. Currently, only the National TB Reference Laboratory of the Research Institute for Tropical
Medicine (NTRL-RITM) in Muntinlupa City is catering to all specimens nationwide. This resulted in prolonged turnaround time
which affected the treatment of patients, hence the need to operationalize the CTRL as the LPA laboratory for Visayas, and
NMTRL for Mindanao. This will ensure that patients are promptly tested, and treated accordingly. Specimens for COVID-19
testing
may be rezoned to nearby COVID-19 testing laboratories.

2. All TB laboratories (see Annex A) that are licensed to operate as COVID-19 testing
laboratories performing cartridge-based PCR shall co-share TB and COVID-19 testing
in the same facility. All laboratory personnel shall observe the biosafety measures
recommended by the RITM (see Annex C — Biosafety guidance for co-sharing of
Tuberculosis (TB) and Coronavirus Disease 2019 (COVID-19) testing in the same
facility).

3. TB laboratories in provinces without any access to RT PCR testing may still apply for
a License to Operate for Cartridge-based COVID-19 Testing but shall operate under
co-sharing agreement (see Annex B).

4. All other TB culture and DST laboratories of the NTP used for COVID 19 testing shall
revert to exclusive TB testing except Ilocos Training Regional and Medical Center,
Cotabato Regional Medical Center, Bicol Regional Diagnostic and Reference
Laboratory, and Teresita Jalandoni Provincial Hospital which are allowed to conduct
exclusive COVID-19 testing until December 31, 2021 only.

Concerned Centers for Health Development, institutions, hospitals and medical centers shall
ensure the timely implementation of these guidelines. The NTP, together with the National TB
Reference Laboratory - RITM, shall monitor the implementation against timelines.

BIOSAFETY GUIDANCE FOR CO-SHARING OF TUBERCULOSIS (TB) AND

CORONAVIRUS DISEASE (COVID-19) TESTING IN THE SAME FACILITY

Mycobacterium tuberculosis (M.tb) is a bacterium that causes TB, whereas Severe Acute
Respiratory Syndrome — Coronavirus 2 (SARS-CoV-2)

through the respiratory route. Therefore, it is a must that a laboratory testing for both TB and
COVID-19 in the same facility adhere to standard biosafety measures to ensure safety of people
inside and outside the laboratory. A combination of engineering, administrative, practices
and procedures, and personal protective equipment (PPE) controls should be
implemented to mitigate the risk of exposure, infection, release, and unintentional spread of
contamination of both M.tb and COVID-19.

The laboratory should meet all the core biosafety requirements related to both TB and COVID-
19 at the minimum (for further guidance, see References). In addition, below are the heightened
requirements that the laboratory must observe:

A. ENGINEERING CONTROLS

1. The facility should be compliant to biosafety level two (BSL2) standards.

2. There must be a certified, class II biosafety cabinet (BSC) where specimens should
always be handled.
3. A dedicated area for donning and doffing of PPE must be available.

B. ADMINISTRATIVE CONTROLS

1. The laboratory must comply and maintain the requirements to operate as both a TB
and COVID-19 testing laboratory.
2. Local risk assessment should be reviewed and updated, and the risk management plan
must be prepared and modified accordingly. The laboratory’s top management should
its
guarantee support for the implementation of the biorisk management program. To
mitigate residual risks, the local risk assessment should identify and document:
a. All contaminated or potentially contaminated items, work surfaces, and other
sources of contamination; and
b. All potential incidents, and emergency situations relevant to biological risks
and product contamination.
3. A specific work assignment and schedule should be well-defined such that TB and
COVID-19 samples are not processed at the same date.
4. Personnel must be trained to perform the different laboratory procedures.
5. There should be an active reporting system for any untoward laboratory incidents.
Adequate contingency and emergency measures must be in place.
 6. An active employees health surveillance and monitoring program specific for TB and
COVID-19 should be in place. Laboratory personnel must be evaluated and treated for
any existing sickness or conditions that make them susceptible to infection.
7. The laboratory’s standard operating procedures (SOP) must be documented, reviewed,
and updated based on the recommendations in this document. These SOPs must be
strictly adhered to at all times.

C. PRACTICES AND PROCEDURES

1. Laboratory workflow and practices following the “clean to dirty” principle must
always be maintained.
2. TB and COVID-19 testing must be scheduled such that they are not performed
simultaneously.
3. Limit the number of personnel inside the laboratory, and restrict access to authorized
personnel only.
4. Different sets of PPE should be worn for TB and COVID-19 testing (i.e. PPE used for
TB testing must not be reused for COVID-19 testing, vice-versa). Ensure proper
donning and doffing of PPE.
5. Verified and appropriate decontamination procedures must be in place:
a. Before and after work, and shifting from TB to COVID-19 processing, vice-
versa, allow the facility’s exhaust system, and the BSC to run for at least an
hour to ensure complete purging of contaminated air.
b. Ensure availability of appropriate chemical disinfectant solutions:
i. Sodium hypochlorite (household bleach), diluted to 1:10 and 1:100 in

water, prepared daily;

ii. Phenolic compound, diluted to 5% in water, prepared daily;
iii. 62-71% alcohol solution.
c. Before and after work inside a BSC, and shifting from TB to COVID-19
processing, vice-versa, decontaminate the BSC’s surfaces as follows:
i. Wet the BSC’s surfaces with either 1:10 Sodium hypochlorite solution,

or 5% phenolic compound solution, and leave for 15 minutes;

il. Afterwards, wipe twice with 62-71% alcohol solution to remove
disinfectant residue.
d. Before and after work, and shifting from TB to COVID-19 processing, vice-
versa, decontaminate laboratory surfaces as follows:
i. Wet surfaces with 1:100 Sodium hypochlorite solution, and leave for
10-15 minutes;
ii. Afterwards, wipe twice with 62-71% alcohol solution to remove
disinfectant residue.
6. When handling specimens, observe necessary measures to prevent the generation of
infectious aerosols.
7. In the designated sample storage space, arrange samples such that TB and COVID-19
specimen containers do not mix.
 8. All wastes generated inside the laboratory should be properly decontaminated prior to
disposal following established healthcare waste management regulations and practices:
a. Used GeneXpert cartridges should be completely soaked in 1:10 Sodium
hypochlorite, or 5% phenolic compound solution for 10-15 minutes;
b. All other wastes must be autoclaved at 121°C, 15 psi for 20-30 minutes.
9. All incidents, including near misses, must be documented and reported. Likewise,
absence of incidents should be documented and reported. Ensure proper management
of untoward incidents in the laboratory:
a. Biological spills must be decontaminated properly;
b. Appropriate intervention (e.g. medical) should be given to personnel involved
in a laboratory accident.

D. PPE CONTROLS

1. The minimum PPE to be wom inside the laboratory are as follows:

a. Gloves (nitrile, and double-gloving recommended);
b. Laboratory gown (disposable, fluid-resistant, long sleeved with cuffs, solid
front, back-tied recommended);
c. Fit-tested NIOSH-approved filtering face piece respirator that provides a level
of filtration of 95% of higher (e.g. N95, its equivalent, or higher respirator).

2. Additional PPE may be used based on local risk assessment:

a. Face shield / goggles are recommended for procedures where splashes to the
face / eyes are anticipated;
b. Dedicated laboratory shoes / disposable shoe covers are recommended to
prevent cross-contamination between the environment, and the laboratory;
¢. Hair cover is recommended to prevent personnel and product contamination.
