ADDENDUM

This Addendum is made to the Student Undertaking, (the ADDENDUM), between PW

Philippines Corporation (the Company) and its students.

Program

株式会社 PW Philippines および、関連会社(ONODERA GROUP)制作の媒体にて、学生の写

真や動画使用の御承諾を何卒、お願い致します。つきましては、下段の写真並びに動画

などの利用に関して内容をご確認の上、ご承諾いただきますようお願い致します。

PW Philippines Corporation advertise and prepare sales materials for customers as

well as the public. It is important for the Company to use realistic and concrete

information to represent our daily business activities.

We respectfully request the permission of the employees to allow the COMPANY to

use photographs and videos to the extent specified below. Provided you agree to the

terms below, please fill in the Declaration of Consent.

利用方法に関して

 株式会社 PW Philippines および、関連会社(ONODERA GROUP)の CM、ホームペー

ジ、社内報、会社パンフレット、広告、チラシ、ポスター、営業資料、メールマガジンな

どにて使用。

 肖像(写真や動画)は、株式会社 PW Philippines および、関連会社(ONODERA GROUP)

の作成物でのみ使用します。

 掲載について削除依頼があった場合は、すみやかに削除いたします。

 上記確認事項は、退職以降も有効とします。

Free of charge to the extent specified below

 The consent would grant the COMPANY rights to use the personal photographs and
videos taken during the course of employment for the purposes of advertising,

 websites, company newsletters, company brochures, and any sales materials
made by the COMPANY in any form.
 The consent also includes permission to mention the employee or student’s name.
 The photographs and videos will only be used in advertisements made by the
COMPANY.
 If there is any formal request for deletion after posting the photographs and videos,
the COMPANY will delete the photos and/or videos immediately, or if not
practicable, on the next working day.
 The above items are valid even after leaving the COMPANY.

個人の写真や映像を使用することに同意します。

I, the undersigned, hereby consent to the use of my personal photographs and videos

made by PW Philippines Corporation.
