SERVICE CONTRACT AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SEVICE is made and executed at the City of Biñan, this 3rd day of October, 2018, by and be-
tween:

UNIVERSITY OF PERPETUAL HELP SYSTEM LAGUNA, a private educational institution duly orga-
nized and existing under Philippine laws, with principal place of business at UPHSL Compound, Old National High-
way, Brgy. Sto. Niño, Biñan, Laguna, represented herein by its School Director, DR. FERDINAND C. SOMIDO,
hereinafter referred to as “UNIVERSITY”.

-AND-

JUSTINO GILBERT B. BELAN, of legal age, Filipino, and with residence and postal address at #50 Li-
maco Tulay Bato, Brgy. San Antonio, Biñan City, Laguna, hereinafter referred to as “SERVICE PROVIDER”.

TERMS AND CONDITIONS

1. DESCRIPTION OF SERVICES. Beginning on October 11, 2018, the SERVICE PROVIDER will provide
the following Services for Mr. and Ms. UPHSL 2018 Pageant:
a. Directing Services;
b. Photo and Audio-Visual Shoot Supervision;
c. Choreography; and
d. Program Flow

2. PAYMENT. Total payment shall be made to the SERVICE PROVIDER in the amount of FORTY THOU -
SAND PESOS (Php 40,000.00) upon completion of the services described in this contract. Partial payment
amounting to TWENTY THOUSAND PESOS (Php 20,000.00) which is equivalent to half of the professional fee
shall be settled by the UNIVERSITY on October 11, 2018 during the scheduled Photo and AVP shoot.

In the case of failure to provide the Services covered in this contract, the SERVICE
PROVIDER shall pay
all costs of collection, including without limitation, reasonable attorney fees. In addition to any other right or
remedy provided by law, the UNIVERSITY has the option to treat such failure to render services as a material
breach of this contract and seek legal remedies.

In the case of failure to pay for the Services when due, the UNIVERSITY shall pay all costs of
collection,
including without limitation, reasonable attorney fees. In addition to any other right or remedy provided by law,
the SERVICE PROVIDER has the option to treat such failure to pay as a material breach of this contract and
seek legal remedies.

3. HANDLERS AND MAKE-UP ARTISTS. SERVICE PROVIDER shall recognize the privilege of every
college or department to hire handlers and make-up artists of choice and shall not make recommendations in fa-
vor of any colleagues or affiliates.

4. JUDGES AND RESULTS. SERVICE PROVIDER will not take part in the invitation of members of the
Board of Judges and will not, in any manner, influence or interfere with judging during the contest.

5. TERM. This contract will terminate automatically upon completion of the Services required by this con-
tract that is after the pageant Coronation Night on October 24, 2018.
