In order to prepare the certificate, DQS requries an informal listing of all company sites participating in the certification process, complete with
address and individual scope in the languages requested. This listing can be added on page 2 or needs to be submitted as an additional annex.

An annex to the main certificate will be prepared upward from four sites. For all company sites listed in the annex, excerpt certificates may be
ordered as shown above.

Based on the signed certificate template and the requirements pertaining to DQS certificates, the certificate will be prepared and dispatched to
the contact person for approval. Changes to an already approved certificate will be invoiced at cost.

All contractual agreements and currently valid prices remain valid.
