I, hereby undertake, authorize, confirm, declare, bound, abide and agree to Kotak Securities Limited (KSL) that:

I have applied and consented for opening a trading and demat account digitally through any electronic means / device with KSL.
In order to facilitate the account opening, I have provided all the necessary details to the Relationship Manager (RM) of KSL

Mobile number __________________________________ and hereby confirm that any and all such details have been entered
digitally by the RM in my presence and with my due consent and authorization. I hereby authorize KSL to register any and all
such details to my accounts with KSL and confirm that I shall not dispute to any such details provided by me and shall be bound
by the same.
I confirm having received/read and understood the contents of the Rights and Obligations, Risk Disclosure Document, Additional
risk disclosure documents for Options trading, Guidance Note including any such documents pertaining to commodities, Policies
and Procedures for client dealings, Additional risk/obligation/liability statement, Standard set of documents etc. in electronic
form, available at: http://www.kotaksecurities.com/ksweb/Important-Policies . I do hereby agree to abide by such provisions as
outlined in these documents. I have read, understood and confirm to abide by all the terms and conditions in the entire account
opening form, Power of Attorney and related documents and I confirm that my below signature be considered as specimen
signature as well as signature on Part A of the KYC form as required by KRA/SEBI. I have no objection to Kotak Securities Ltd
cropping my signature and pasting it on the account opening documents for completing the account opening process. I wish to
proceed with the account opening without providing a copy of my signature proof. In case I submit any physical request post
account opening, I agree and confirm to provide the signature proof copy and other necessary documents in order to facilitate
the same. I am aware that my signature is required only on MITC, Power of Attorney and Nominee declaration form and I shall
not dispute to any of the Data/details and other terms and conditions for the reason of not signing on the KYC/Account opening
form and/or any other such documents. I agree to abide by the Bye Laws and Business Rules of NSDL. I have read and understood
the current rules and regulations of the Depository and the Depository Participants pertaining to an account, and I agree to abide
by and be bound by the same. The details furnished through the above process of digital account opening are true and correct to
the best of my knowledge and belief and I undertake to inform KSL of any changes therein, immediately. If the above information
is found to be false, untrue, misleading or misrepresenting, I shall be held liable for it.

Office use : I undertake that I have made the client aware of ‘Policy and Procedures’, tariff sheet and all the non-mandatory
documents. I have also made the client aware of ’Rights and Obligations’ document(s), RDD and Guidance Note I/We undertake
that any change in the ‘Policy and Procedures’, tariff sheet and all the non-mandatory documents would be duly intimated to the
clients. I/We also undertake that any change in the ‘Rights and Obligations’ and RDD would be made available on my/our website,
if any, for the information of the clients. I/we confirm that all details provided on sales app are as provided by client only. I have
met the customer in-person and interviewed him. I have verified the documents provided by customer with original.
