Objectives: At the end of the home visit, the family will be able to:
1. Understand the significance of having postnatal care
2. Identify factors that affects the sudden gush of blood
3. Practice necessary self-care that is being recommended by health
professionals.
Plan of Activities
1. Greet the Client, Introduce yourself
and Verify the client’s identity
2. Explain to what you are going to do
throughout the procedure
3. Provide privacy to the client
4. Wash hands
5. Assess the Vital Signs of the client.
7. Ask the patient regarding to her
vaginal discharge after giving birth.
8. Ask the client about the details,
characteristics, and approximate
estimation of blood loss.
6. Explain the significance of having
regular postnatal check ups.
7. Indicate the various factors that affects
the mother from preventing to seek
health services.
8. Emphasize life-threatening
complications related to heavy bleeding
post vaginal birth.
9. Discuss the findings to the client.
10. Document the findings in the client’s
record.
Rationale
- To establish comfort to the client’s state.
- Explaining promotes client’s
cooperation and put the client at ease
- To ensure that the patient feels safe
and promote effective communication to
the client
- To prevent cross contamination
- To provide baseline data and check the
client’s physiologic health status
- Lochia has three stages. The first three
days after delivery is dark red in color.
For the fourth through tenth day after
delivery, the lochia will be more watery
and pinkish to brownish in color. About
two weeks later the discharge may
become light brown or yellow in color and
the amount will be much lighter.
- To gain subjective cues of the chief
complaint of the patient which will serve
as one of the basis in identifying the
problem.

it important to receive health care

patient from seeking postnatal care.
- To recognize conditions that may
related to the client’s situation and help
prevent to happen by implementing
effective interventions.
