FIRST ROUND SCHOOL-BASED DEWORMING TABLET ADMINISTRATION

FOR S.Y. 2022-2023

To: Assistant Superintendent

Education Program Supervisors/Coordinators
Public Schools District Supervisors
Public Elementary and Secondary School Principals/Heads
All Others Concerned

1. This Office disseminates that the Deworming Activity for School Year 2022-2023 will be conducted
on September 26 to 30, 2022.

2. In view of the aforementioned, all School Health Coordinators shall coordinate and facilitate
compliance of the following:

a. prepare the masterlist of enrolled public school students for the easy identification
and accounting of recipients of the deworming services;

b. prepare the advocacy leaflets from DOH CVCHD "Mga kinahanglan nimong
mahlbaloan mahltungod sa Soll Transmitted Helminthiasis" and to secure the
accomplished parent's consent form with strict adherence to the "NO CONSENT, NO
DEWORMING POLICY". Both the advocacy leaflet and the parent's consent form will
be given during module distribution/ face to face classes and shall be collected prior
to Mass Drug Administration (MDA);

c. ensure that students who were given deworming tablet by their Barangay Health
Workers last July 2022 shall not be included in this first round deworming activity.

3. Schools still having limited face to face classes, the deworming may be done at home to be
administered by their parents/guardians.

4. All School Health Coordinators are advised to secure the deworming tablets from the Division
Health and Nutrition Unit Office on September 19, 2022 from 8:00am to 5:00pm c/o Dawn Marie L.
Tagimacruz or any health personnel on duty.

5. A soft copy of the consolidated list of learners dewormed shall be submitted via email at
dawnmarie.tagimacruz@deped.gov.ph on or before October 14, 2022.

6. Immediate dissemination of and compliance with this Memorandum is directed.
